I am always thrilled to bring on another gut specialist and Dr. Ilana Gurevich was so gracious to appear on the podcast this week.

She is a licensed acupuncturist and Naturopathic Physician in the Portland, Oregon area. She has a clinic called Open Wellness Pdx where she specializes in all things gut health. In addition, she hosts a new podcast called The Turd Nerds . And, she is the President of the Gastroenterology Association of Naturopathic Physicians (GastroANP) which is a board certification and continuing education organization for digestive health focused naturopathic physicians. Finally she is an instructor in Neural Therapy at Learn Neural Therapy.

We had a delightful conversation about some areas that are emerging in digestive health such as the virome, the enteric nervous system, mitochondrial health, phage therapy, biofilm, small intestine microbiome, and the impact of ferments on food.

When people are anxious, they often get sweaty hands, increased chest pressure, shortness of breath, feel lightheaded, and they may feel tingling sensations in different parts of their body.

By paying attention and having awareness to these symptoms and , they can start to calm down a little bit and self-regulate.

Yet when stuck in this loop of anxiety , people often turn to learned coping behaviors or addictions to deal with these unwanted sensations.

Learning to deal with this pressure cooker , before it turns into dysfunctional behavior, is something that is the work my guest Eva Angert Harren.

And according the Eva, dealing with the pressure cooker does not require changing your thoughts or trying to control things. Actually its quite different; it's more about the being with the body/somatic experience and learning to release it.

Current studies estimate that 2.7% to 6.5% of women in the United States have symptoms consistent with a diagnosis of interstitial cystitis/bladder pain syndrome (IC/BPS). The broad range in prevalence depends on whether a high-sensitivity definition (6.53%) is used versus a highly specific definition (2.7%)
And that is really an underestimate, because most people complaining of painful, urinary discomfort,  urgency, and frequent urination; in the absence of an infection are just brushed off by their health care provider and sent home with no solutions. Meanwhile they suffer, with constant urging to urinate and pain sometimes to the point that urinating feels like peeing chards of glass.  It is not something that is over in a few days. This is a chronic and intermittent condition  In previous episodes of the one thing podcast we spoke about IC and got a nice view of the
microbial  and also biofilm component; however there was more to the story that I wanted to explore. So I reached out to Dr. Mandy LaGrega who is known as the IC healer.  I was intrigued by her work in this area especially when it comes to her knowledge of the Oxalate component of IC/PBS and her understanding of the gut, fungal, mycotoxin, and psychoemotional role of these disorders.  We had a great conversation on this episode about how IC/PBS develops, how oxalates play a role,  the risk factors for IC/PBS, and overall approaches to healing this disorder from a nutritional approach.  Enjoy!

Mark Pimentel, MD, FRCP (Fellows of the Royal College of Physicians – Canada) Mark Pimentel, MD, is a Professor of Medicine at Cedars-Sinai. Dr. Pimentel is also the Executive Director of the Medically Associated Science and Technology (MAST) program at Cedars-Sinai, an enterprise of physicians and researchers dedicated to the study of the gut microbiome in order to develop effective diagnostic tools and therapies to improve patient care. Heart Rate Variability (HRV)  is a key metric in the overall homeostatic balance of the body.
HRV is simply a measure of the variation in time between each heartbeat. This variation is controlled by a primitive part of the nervous system called the autonomic nervous system (ANS).  It is largely an indicator of how well the body is handling the Allostatic Load (stress load).

I didn't truly grasp HRV until I was exposed to the work of Dr. Marco Altini; so I decided to ask him to come on and help educate my listeners.

Slowing aging , preserving memory, and preserving energy are big topics of concern in medicine and health care. Over the past 10 years research and development has show a special interest in how to modify Sirtuin-1 as a protein for longevity and slowing the disease process.  A cofactor of Sirtuin-1 named Nicotinamide Adenine dinucleotide  (NAD) is of particular interest.    NAD seems to be depleted as we age and thus turning down Sirtuin-1 activity.  This means accelerated aging.   The study of how to modify NAD is blossoming much thanks to the work of David Sinclair, Phd. He is Research Investigator at the Helfgott Research Institute and Visiting Research Scholar at the University of Technology Sydney. o Dr. Goldenberg is a passionate educator and is faculty for the Academy of Integrative Health and Medicine’s Interprofessional Fellowship in Integrative Health and Medicine. 

 He is past adjunct faculty at Bastyr University, his alma mater, in which he enrolled after receiving honors and distinction in molecular biology from the University of Pennsylvania. 

 As a naturopathic doctor Dr. Goldenberg focuses on integrative approaches to irritable bowel syndrome as well as other gastrointestinal complaints at Goldenberg GI Center.

Myalgic Encephalomyelitis/Chronic Fatigue Syndrome (ME/CFS) is a debilitating condition with symptoms of persistent fatigue, neuroinflammation,  post-exertional fatigue, joint pain, nerve pain, muscle pain, sensory hypersensitivity, disturbed sleep, headaches, and bowel changes.

It often takes years to get a proper diagnosis and most patients are brushed off as having a psychosomatic illness.  '

Thanks to researchers like our guest Dr. Amy Proal; we know so much more ME/CFS. It turns out there chronic viral and other microbe components that drive a significant part of ME/CFS. These pathogens/pathobionts can set up shop and change DNA, mitochondria, and immune function that contributes to the manifestation of ME/CFS.

In this episode , Dr. Proal dives into how viruses find tissue to hide out in, disrupt mitochondria, alter the vagus nerve and how this understanding guides us how in treating ME/CFS

Menopause with Dr. Ginger Nash

"Puberty in Reverse" .

That's what Dr. Ginger Nash, refers to the menopausal transition leading into the menopausal years.

Even as a guy, that statement really drills down what the menopausal stage can be for some.

With anxiety, body composition change, irregular menstruation, and sleep disturbances often presenting as some of the early signs of the menopausal transition,   it can be a distressing time for many women.

According to Dr. Nash, only about 10% of women will sail into the menopausal years with no symptoms or major physiologic balances.

She speaks about the factors that make the menopausal transition particularly rocky and some ways of approaching those years that is quite refreshing and opening. 

And she shares that going through menopause can be supported with lower force interventions and does not necessarily need to involve bio-identical hormone replacement. However, that option may be appropriate in certain situations.

Dr. Nash earned her doctorate in 1998 from The National University of Natural Medicine in Portland, Oregon. She owns and operates a general medical practice in New Haven, CT that specializes in women's health and chronic immune dysregulation. Dr. Nash has also lectured and taught healthcare programs throughout the U.S. and Canada for practitioners of natural medicine for over two decades. Polyphenols fall under the category of phytonutrients.  Which are plant nutrients that confer health benefits.  Polyphenols specifically are food for our microbiota and are involved in housecleaning of potential pathogenic pathobionts.  We should really strive for about 1000mg-1500mg of polyphenols in our diet daily as the evidence is stacking up for their  health benefits for colon health, brain health, cardiovascular health, and overall inflammation support.  In this episode of WIALD (what I am learning daily) I go into the best sources and further unpack polyphenol benefits.
