Living with the consequences of severe birth injury with Joanna Prance
**** Trigger warning This episode discusses birth trauma and birth injuries*****
Perineal injuries during childbirth known as OASI (Obstetric Anal Sphincter Injury) or 3rd and 4th degree tears. A third-degree tear is a perineal tear that extends into the muscle of the external anal sphincter. If the tear extends further into the lining of the anus or rectum it is known as a fourth-degree tear. The incidence of OASI in the UK is 2.9% with an incidence rate of 6.1% in first births – meaning around 1 in 20 first-time mums in the UK suffer this type of serious injury. The main risk factor for anal incontinence amongst childbearing women is an OASI injury but OASI injuries can also cause other symptoms of pelvic floor dysfunction. An OASI can have a devastating impact on quality of life - affecting a woman’s health, intimate relationships, family relationships and employment.
In today's episode Emma Talks to Joanna Prance, Personal Trainer, Yoga Instructor, Ostomate and a Mum. In 1998, Jo was 22 when she sustained life-changing injuries during the traumatic birth of her son. The extent of these devastating injuries only manifested over time and she could never have imagined the ordeal of what the subsequent 23 years would bring or the impact her childbirth experience would have on the rest of her life. As a result she has undergone multiple treatments, procedures and over 17 surgical interventions, including TVT mesh, sacral neuromodulation and colostomy surgery.
Speaker Bio
Through her work as Speaker and educator Joanna is keen to raise awareness of how life-changing severe childbirth injuries are to those who suffer them and their families and to improve the outcomes for those affected by these injuries.
Jo began her involvement with The MASIC Foundation in 2019, a charity that supports women who have suffered serious injuries like OASI (Obstetric Anal Sphincter Injury) during childbirth and is a MASIC Advocate.
She has also been engaged with project work for Colostomy UK, a charity that supports and empowers people living with a stoma
A keen sportswoman, she has sustained an active lifestyle despite facing numerous challenges as a result of her injuries. She enjoys yoga, walking, strength training and last year returned to competitive swimming.
Jo’s fitness and healthcare career spans over 17 years through her private work as a Personal Trainer and formerly in the public sector as a specialist Cardiac Rehabilitation Instructor.

Is there such a thing as an optimal birth? Changing the conversation with Molly O'Brien
In this episode Midwife Molly O'Brien joins Emma and Grainne to discuss childbirth. Molly has seen many women and birthing people suffer long and difficult births which led her to explore physiology and the biomechanics of human movement and apply it to the birthing process. She discusses the disruptors of the physiological birthing process and how women and birthing people can optimise the birth process. This is a great episode for anyone that works with pregnant women, is pregnant or has a pregnant friend or relative.
To find out more about Molly and her course visit www.optimalbirth.co.uk
Molly O’Brien is an experienced, registered midwife whose 24 years of practice is guided by a salutogenic approach to birth with an emphasis on preserving, supporting and optimising the physiological birth process to achieve the best possible birth outcome and experience for the mother and her baby.
She created and teaches a course for midwives, doulas, obstetricians and birth associates called ‘Biomechanics for Birth’. The course has evolved from a combination of clinical midwifery, an autodidactic pursuit of knowledge about biomechanics from sources including physiotherapy, osteopathy and movement specialists and a career attending more than a thousand births, refining the midwifery skills of perception and observation of movements, positions and attitudes of birthing women.
Attending hundreds of undisturbed physiological births in the home and midwife led birth unit setting gave her the opportunity to make a systematic exploration of midwifery skills and observations that can help identify the reasons behind labour dystocia (difficult labour).
When labour dystocia is caused by a mechanical imbalance it requires a mechanical solution using specific and directed movement. The techniques and strategies that emerged from this time helped her to revolutionise her practice and she began to regularly witness rapid and dramatic progress as she put them to use during “difficult labours”
Her aim is to introduce an understanding and resolution of the root cause of mechanical labour dystocia into mainstream midwifery and obstetric practice and education of maternity health care practitioners. There is a real possibility to significantly reduce intervention, increase straightforward births and improve the birth experience.
Molly teaches the principles of biomechanics in the birth setting as part of a Masters degree in two universities in the UK and in a Chilean midwifery school. She is regularly invited to teach in NHS hospitals and she frequently teaches the course online to a global audience of multidisciplinary practitioners.

Why Sleep Matters with Matt Philips
Why is sleep so important? What effect can a lack of sleep have on your mental and physical health? Can caffeine really affect your sleep that much? What can you do when sleep is hard to come by?
These are the type of areas around sleep that we delve into with Matt Philips this week.
For over ten years, Matt was part of a multidisciplinary team of physiotherapists, sports therapists, osteopaths, podiatrists and massage therapists. In witnessing the coming & going of many misconceptions within the Running Industry, Matt decided through his website www.runchatlive.com to try and help both runners and therapists evolve from outdated ideas and embrace a more evidence-based approach to running injury prevention & management. Matt has written for numerous websites & running magazines including Runners Connect & Outdoor Fitness. Matt currently offers specialist online consultation for runners & non runners struggling to achieve results. The ‘Sports Therapy Association Podcast‘ is recorded live every Tuesday at 8pm (UK time) via YouTube then upoaded to all podcast podcast apps.
Matt is not a sleep specialist but has suffered from insomnia and sleep deprivation for years. Keen to understand more this year he underwent a sleep study. Modern sleep research is now far more equipped at measuring the effect of poor sleep quantity & quality, and the results are pretty shocking as Matt Philips, has found.
Join us as we discuss many of the answers to the questions posed above and explore just why sleep matters.

My Chemical Menopause with Kirsty Dougal
Kirsty Dougal is Scottish but lives in the Cotswolds. She is 42 and works in research funding for biological sciences. Outside of work, she is passionate about horses, having previously worked in the equine industry. If money was no object she would happily spend her time outdoors cuddling horses! 4 years ago her life was turned upside down due to the perimenopause and menopause. She experienced the perimenopause at 38! Her symptoms were life-altering and her battle to seek diagnosis and treatment have been very difficult. Her transition into the menopause is not conventional. At 41 she went through a chemical menopause before having surgery last year to be put into a surgical menopause! She realised first-hand how poor support is and is now driven to do what she can to help other women understand the topic and know how to advocate for themselves and access the right help.

Why Mum's Don't Jump! Pelvic Organ Prolapse with Helen Ledwick
We open this season talking to Helen Ledwick the creator of ‘Why Mums Don’t Jump’ -- a podcast about pelvic floor problems after childbirth. She’s a former BBC journalist and a mum-of-two with pelvic organ prolapse.
When delivering her second baby, Helen experienced a 3rd-degree tear and subsequent pelvic organ prolapse. Unaware of pelvic floor dysfunction until this point and frustrated, anxious, and keen to learn more, Helen began to explore the world of pelvic health and found that there were so many more women affected by these common but not normal symptoms that can occur at any time in a woman life, but often around pregnancy and childbirth.
Helen 'fell' down many rabbit holes whilst researching her symptoms and was overwhelmed by the misinformation, scaremongering and myths that existed for all pelvic floor conditions including pelvic organ prolapse. Season 2 Ep 7 Why is laughter the best medicine? with Elaine Miller
It’s taken us a whole year to get this super talented but super busy lady onto our podcast but it’s finally happened, just in time for Christmas!
Elaine Miller has impressively combined art and science, to create comedy sketches and shows that break down barriers and educate men and women about their pelvic health.
You may have seen her on @lorraine, @packedlunchc4 or even heard her on @bbcwomanshour, talking about pelvic health in her own hilarious and approachable way!
Her humour is infectious and approach is unique. We were super keen to delve into her own personal experience of urinary incontinence, the work she is doing in schools, comedy clubs, the Edinburgh Festival, with media, and in Parliament!!! We also wanted to find out why Laughter is the best medicine even in the world of pelvic health!
Elaine Miller is a fellow of the Chartered Society of Physiotherapy and award-winning comedian. She is interested in the sociology of incontinence and the barriers to help-seeking. Her paper on using humour as a public health promotion tool was published in August and she is about to tour her comedy show "Viva Your Vulva" to further that research.

Season 2 Ep 6 Midwifery, Motherhood and the Maternity Crisis with Midwife Pip
This week Emma and Grainne speak to Pip. Pip is a practicing NHS Midwifery Sister in the UK, an enthusiast of all things women’s health and also mum to her 9-week-old baby boy. Pip has worked across various maternity settings from community, homebirth, birthing centres and is now based on a labour ward (although she is currently on maternity leave). Now she has had a baby we were fascinated to find out what her views are of midwifery and of course motherhood now she is on 'the other side'. We talk frankly about the 'maternity crisis' and explore the changing landscape of pelvic health and collaborative work necessary for effective perinatal care.
Pip believes that pregnancy and birth are such monumental times in a woman’s life that should be truly positive and empowering regardless of the twists and turns that may crop up. Based on this, Pip’s mission is to provide honest support and information through her Instagram page @midwife_pip, Podcast- Midwife Pip Podcast and through her online courses which can be found at www.midwifepip.com.
Season 2 Ep 5 Getting physically active and exercising during pregnancy with Dr Margie Davenport
Advice and guidance have dramatically varied over the years around this topic. In the mid 1900s pregnancy was regarded as a state of confinement. It was believed that exercise should be discouraged for fear of harming mother or growing baby.
Thankfully things have changed and now women are encouraged to be physically active and exercise because of all the wonderful benefits they bring both mum-to-be and baby. However, there still remains uncertainty in this area. In 2019 a team in Canada led by Dr. Margie Davenport wrote much-needed evidence-based guidance - The Canadian guidelines for physical activity throughout pregnancy. This guidance details why and how women can remain or even start to get physically active during their pregnancy.
In today's episode, Margie talks to Emma and Grainne about the guidelines, the benefits of physical activity and exercise, busts many of the myths around this topic and discusses the need for further research so that women are better served during their pregnancies.
If you are thinking about getting pregnant, pregnant or working with pregnant women this episode is for you.

Season 2 Ep 4 'Beating Cervical Cancer - My Story' with Sarah Newman
Season 2 Ep 3 'Decolonizing Fitness' with Inemesit Graham
This week Grainne and Emma are joined with Inemesit Graham a fitness professional, pregnancy, and postpartum athleticism coach.
In season 1 we spoke with Illiyin Morrison, a midwife and birth trauma specialist who regularly highlights and educates about racial inequalities in healthcare. Today we wish to build upon that conversation by delving deeper and gaining wider perspectives from Inemesit Graham.
Inemesit was born in Nigeria, moving to England when she was 5 years old, and has currently resided in Canada for the last 13+ years.
She is the founder of Mummy Fitness, an online perinatal fitness platform and her passion for health and fitness stems from her own struggles of understanding her body. Body image issues trace back to early childhood when she moved to the UK, highlighting the societal expectations and constructs created in the western world. Her journey into motherhood further challenged body image and expectations. The more Inemesit began to learn and understand about her body, the less she disliked it. She is now a voice of self-appreciation and racial equality and inspires women all around the world. Her goal is to offer a decolonized approach to fitness based on body liberty and not body oppression.
