0-5 Introduction of Mike Boyle . Should high school athletes turn back to sport between 6-9 months mark post operative ACL repair? The data is clear, earlier return, higher chance of re tearing. Understanding importance of the ability to decelerate. Not ready to return until athlete can decelerate under high force. The problem with "decel drills" being used and not testing an individual under running 100%. Interpreting the FLY 10 TEST. Looking at athlete's best vertical jump.

5-10 The single leg hop and triple leg hop test is antiquated and not practical. How physical therapy is going backwards as a profession with sports performance. Poor criteria for return to play.

10-15 Machine company pushing products and influencing rehab and training environments. Leg extensions are not as important as being implied. Plenty of functional exercises that will isolate the quads. The poor status and presentation of athletes being discharged from physical therapy. Go to exercise is the split squat and assault bike sprints! Athletes should not be in pain during rehab!

15-20 The problem with the Biodex and isolated testing as a hang the hat on metric. Considering multiple factors. Really diving into the "research" and looking at bias and self funded researching, old school "quad and hamstring" talk.

20-25 Understanding why functional movements are important for sports performance and translating into the sport. ACL multi factorial, considering the turf and the shoes. Turf and shoe companies want no responsibility for contributing to tears. Injuries still on grass, still overall issues with poor training even at professional level. Importance of unilateral hopping bounding and progressions. 10 single leg squats with 30% of bodyweight.

25-30 Importance of single leg jumping towards the mid line. Understanding female puberty contributing to injury. Gaining weight in breast size and wider hips but not enough muscle mass. Wrong shoes on wrong surfaces, Body position affecting injury in sports, field hockey vs soccer and lacrosse. Knee bent position. Problem with "all purpose" cleats. Rehab needs to get better. Approaching 4 month mark and starting to progress into strength and conditioning.

30-35 Poor programming in therapy and wasting time. Understanding growth spurts and proper progressions to establish check points and getting strong at weak links, no away around it. Plyometric progressions, able to single leg hop 5 times over a 12-inch hurdle. The ability to demonstrate "athleticism".

35-40 Power and reactivity, strength and deceleration techniques. Understanding good trainers and good physical therapists. What it takes to be in a late stage return to play setting. CSCS vs. CFSC certification. Book application vs. practical.

40-43 Close Out

Of particular relevance, noncontact ACL injuries have been associated with lower neurocognitive function in non-concussed athletes, which suggested that neurocognitive differences may be associated with the loss of neuromuscular control and coordination errors leading to increased risk of ACL injury.

Question asked by a mom in the "Parents of the ACL Recovery" page

My daughter is 5.5 months post op, quad graft, and went for her 6 month check up today. They ended up testing her for return to play, and she passed with a score of 98%, meaning her surgery leg is almost equal to her other leg! I thought I would post videos of some of the tests that they did in hopes they will help someone. They also did a basic quad and hamstring strength test, and she was equal on those.

Now we are waiting on the dr. to call to let us know what exactly this means. She is a 15 yr old ECNL soccer player and has been cleared by her PT to do everything EXCEPT contact for the past few weeks. Just curious what “return to play” actually means, as I know she will not be playing in games this early!

𝗥𝗲𝘁𝘂𝗿𝗻𝗶𝗻𝗴 𝘁𝗼 𝗦𝗽𝗼𝗿𝘁𝘀 𝗣𝗮𝗿𝘁𝗶𝗰𝗶𝗽𝗮𝘁𝗶𝗼𝗻

Goals at 9-12 months: PREPARE TO TAKE RETURN TO SPORTS TEST

- Display a normal running pattern that does not increase pain, swelling, or warmth

- Practice and display no hesitation or compensation strategies during agility drills (particularly when decelerating) when performed at 100% effort

- Practice and display normal loading (no medial knee collapse) and soft, athletic landings from all jumps and hops

- Practice and display no hesitation or compensation strategies during cutting drills (particularly when decelerating) when performed at perceived 100% effort.

“Return to Play”
it is necessary to differentiate between various types of return:

Level 1
(10) NON contact practices-stretching, warm-up, skills and drills. No more than 1 practice per day

• Return to reduced team training practice (no contact)


Level 2
(10) May participate in Contact or Possesion skills/drills. No scrimmage or games. No more than 1 practice per day

• Return to full (normal) team training practice (with contact)


Level 3
(5) 10 minutes continuous play before resting. Total 20 minutes

• Return to “friendly” games (initially not over the full duration of a match)


Level 4
(5) Increase continuous play time by 10% as tolerated

• Return to competitive match (initially not over the full duration of a match)

Level 5
Athlete can play in multiple games per day

0-15: Who DorsaVi is, how long they have been around and how Dr. Trent Nessler implemented research backed tests to be used with DorsaVi assessments. Joanna’s history and how she became involved with DorsaVI as well as her background as a physical therapist.

15-30: Athletic Movement Index (AMI) Battery of tests takes 15-20 minutes. What patients are appropriate for the AMI testing, what tests are performed and at what stage in rehab they are appropriate. DorsaVI provides video analysis and a report printed out with the objective data. Different athletes and positions require different treatment programs. Current objective measures in the physical therapy profession are lacking efficiency. The profession needs to adapt and move forward with more advanced objective measures.

30-45: The wearable sensors can be used for multiple positions and even outside running. Some agility drills can be completed with the sensors on. The method behind how the sensors are applied and the easy to apply system. DorsaVi can be used in multiple patient populations to analyze movement.

45-60: How the system can be used for analyzing teams and illustrate the importance of athletes requiring a stronger foundation. The difficulty behind playing multiple sports and specialization early on is detrimental towards development of adolescents. DorsaVi can assess baselines throughout growth spurts and changes in body mass and height. DorsaVi is very portable, minimally cumbersome and easy to use anywhere. The report prints in 15 minutes. Physical therapy is slow to change and really needs to be proactive towards forward thinking.

Today we talked with Trent Nessler, Sports PT, Movement Specialist and President of Rebound Vitality.

0-15: Introduction and background on Trent Nessler from his start with working with ACL reconstruction patients and his venture towards creating a system used to help analyze movement criteria with advanced technology.

15-30: The problem with current Return to Sport testing, the hot topic involving the lack of standardization of criteria, the issues regarding reliance on the “good side” for Limb Symmetry Index (LSI). Each athlete and each sport requires movements and stressors that act differently on each person which limits normative value data. Physical Therapists need to get more involved in the research and question current treatment approaches.

30-45: Blood Flow Restriction Training (BFRT) can help with recovery following surgical procedures and Trent explains his first hand experience with using BFR. ACL protocols drastically differ across regions of the country and lack standardization. Kinesiophobia can be reduced by introducing early on single limb training. The severity of deficits amongst the current adolescent sports population. The importance of nutrition and hydration education for performance and rehab.

45-60: The danger of fixating on “return to running”, time based criteria

50-60: How Blood Flow Restriction works and its benefits for healing, performance and improving outcomes.

60-75: Athletes that can move better are at less risk for injury and concussions. Neuroplastic changes occur following major injury such as ACL requiring neuromuscular training and less reliance on visual input to restore the sensory awareness function. How BFR can help for training and maximizing time with exercise choices. Types of BFR cuffs and parameters for compression.

75-90: Trent’s biggest obsessions and keys for success.

0-15min: Introduction and background on Dr. Sewards, his experience in the military, athletic background and progression into becoming an orthopedic surgeon for high level athletes 


15-30 Implementation of Blood Flow Restriction for recovery and performance, strength measures compared to neuromuscular control and position/shape/movement expression and its relation to the appropriate time for return to sport related function, appropriate time for return to running, maturity of the ACL graft and criteria based metrics vs. time based criteria 


30-45: The epidemic of injury and organized sports, early specialization and its relation to injury,  multidimensional athletes, is 3 clubs of the same sport worth it? Time to rehab and focus on foundational building. 


45-60: Burnout and the push for specialization, outdated performance metrics for soccer and field sports, complexity of the ACL surgery, making rehab fun and allowing for a mental break from the sport and enjoying training/fitness again. 


60-75: Recovery metrics, Whoop strap example, nutrition and it’s contribution towards healing, ACL graft types and surgical methods, differences between the grafts and what the research/clinical outcomes demonstrate. 


75-90: Is wearing a brace post ACL beneficial? Complexity of knee injuries related to non contact vs. contact mechanism of injury. Educating the masses on the importance of understanding proper rehab and injury reduction, building a strong healthcare network and getting people into the right healthcare/rehab systems, continuing education and growing with learning.

My 16 y.o. son is 4 months post Surgery from an ACL and meniscus repair. His PT said it was time to ask his ortho for a sports brace fitted for activity. He went to his ortho doc who said he didn’t need a brace. He said they don’t typically fit braces for ACL repairs but only do it if it makes the patient feel better. My son is a soccer goalie, so playing without a brace sounds great. Obviously we’re not there yet. My son has been cleared to run, bike, juggle, and light passing. So, is the work now just strengthening the leg? At what point did you stop PT and then did you enlist a trainer to get your kiddo game fit?
