Chronic Kidney Disease is a global health problem. The biggest issue lies in its late diagnosis during the progression of disease. The clinical manifestation & plasma level indications such as eGFR that can indicate kidney health are reduced only at advanced stages of disease development. However the fact is the changes in biology & chemistry at molecular level takes place years before the nitrogenous substances such as urea & creatinine accumulates in the blood. Unfortunately, up till now, we had no models in place to detect the early signs of chronic kidney disease. Even some of the companies are tracking certain vital parameters such as weight monitoring, sleep, water tracking, nutrition & more but these practices are just focussed on managing kidney disease. It is no brainer that increased risk of type 2 diabetes or high blood glucose level could damage blood vessels of kidneys & increase risk /trigger chronic kidney disease. However, research & empirical evidence has now found that functions of oral & gut microbiome have a role to play in early onset of chronic kidney diseases & could be used as early indication & offer preventative solutions -Halitosis(bad breath) via oral microbial metabolites such as oral H2S, ammonia, polyamines & urease pathways is an early sign. -Secretion of gut microbial derived metabolites & uremic toxins such as p-cresol, Indoxyl Sulphate, TMAO & LPS could impact kidney function.High circulatory level of these toxins could lead to oxidative stress. These recommendations are based on population studies which considers you as an average healthy individual & metabolising food/protein in the same way others metabolise? These are wrong claims as our biology is as unique as our fingerprint. Metabolic disease is taking a giant leap forward with the number of people developing obesity, type 2 diabetes & more is increasing. Although we have all resources & tech in place, still we do not have any preventative solutions to stop these diseases from developing in the first place. There is need to understand -Food processing, farming practices & industrial practices to improve the shelf life: Although labels will show the same value, processed foods such as rolled oats or commercially grinded nuts would be processed differently. Besides, it is also important to decode the functions of your microbiome as 99% of biological functions are performed by them. The interaction between food & microbiome also determine how each food is processed, how much calories are extracted what byproducts is produced as a result - Glycemic response to various foods: Over the last 2 years, we have seen a number of companies coming up with continuous glucose monitoring machines but there are multiple structure & functional issues associated with CGM. CGM are not more that a guess work -CGM measures your glycemic response to various foods post its consumption, does not predict glucose spike -CGM measures response to a meal, not food, not specific molecules in food. We really need to understand how each of molecule contribute to our blood sugar response -You may fix CGM for 2-3 weeks to make a list of food that is going to spike more than others & may consider it static for you. However, as you feed your microbiome, functional pathways & composition changes, glycemic response to food changes & therefore you need to track glycemic response every month, making you endlessly spend on CGM every month. -CGM does not tell you why you have a glycemic response to specific foods as it does not decode the interaction between food you eat & your gut/oral microbiome. - Rather than just tracking glycemic response using CGM, the focus is on understanding the level of production of nitric oxide(which is a signal that tells GLUT 4 to clear the glucose from the circulation, LPS Production, TMAO, P-cresol & more. Technology has taken leap & bounds over last few years still when it comes to nutrition as medicine, we get inclined towards that lucrative quick fix without understanding our unique biology & chemistry “ Oh, have 25 grams of veggies daily! Which one is Broccoli? Tomatoes? Bell Pepper? Carrot? Broccoli - really? What if my microbes producing hydrogen Sulphide are active? What if it could cause bad breath YOur body needs a lot of antioxidants, take pomegranate! Some facts 1)Globally,41 million people die of chronic diseases every year 2)55 million+ people suffer from dementia worldwide 3)Global incidence of Non- Alcoholic Fatty Liver Disease(NAFLD) is 47 for every 1000 people 4)100 million plus Indians are either prediabetic or suffering from type 2 diabetes 5)13% of Indian population have high risk of cancer 6)10% of Indian population suffers from chronic kidney disease; Renal failure is detected at a late stage & most of the patients end up in dialysis with no curative solution 7)Diseases such as NAFLD, Oral & Throat Cancer & Dementia are hard to detect & the list goes on Still these new age technologies in the name of AI & ML are just helping us manage the symptoms of chronic diseases not finding or identifying asymptomatic signatures predictive of chronic diseases & biggest thing these models have missed out tracking 99% of biology AI/ML is a system & we need to feed the right set of data sets to draw meaningful outcomes that could help us understand the pathogenic process involved in onset & progression of chronic diseases. We need to focus not on plasma level biomarkers & DNA but on downstream functions via RNA & Metabolites We need to collect a lot of molecular & biological data sets & apply AI/ML tools to deep dive on what is happening at molecular level & changes in chemistry before & during disease onset. This could help us identify molecular indications which are invisible & cannot be identified by POC diagnostics/Wearables & compute interventions to kill these molecules so that disease does not even develop in the first place. Sexual health issues among male population, specifically, Erectile Dysfunction(ED), has become one of the major chronic health conditions with over 50% of male population in the age band of 40-70 years facing ED. Research has found that men having obesity or suffering from type 2 diabetes &/or high blood pressure are at high risk of Erectile dysfunction. Besides, chronic stress can also lead to ED. All these are interconnections with a common underlying cause- Nitric Oxide(NO) Deficiency. NO is the most important gas produced inside our body which plays a key role in cellular communication, acts as vasodilator, regulates blood flow, oxygen uptake & delivery & facilitates insulin signalling. Infact Erectile Dysfunction is caused by lack of vasodilation & blood flow attributed to NO deficiency NO is produced through two pathways -Endothelial Cell : ENOS, enzyme when coupled with Endothelial cell produces NO by metabolising L-arginine in presence of oxygen. L-arginine is a semi essential amino acid produced inside our body through the Urea Cycle. We do not need to consume excess L-arginine as it could activate Arginase Enzyme & NOS could produce Superoxide, causing oxidative stress. Oxidative stress, inflammation, high glycemic diet as well as oxidation of Tetrahydrobiopterin(BH4 to BH2) which uncouples ENOS, thereby shutting down NO production. As we age ENOS’s ability to produce NO reduces by 10% every year & therefore by the time we reaches age of 50, ENOS ability to produce reduces by 50% & therefore second pathway becomes more important Oral microbiome triggers Nitrate-Nitrite-NO pathway: Microbes in our mouth can metabolise nitrate in green leafy vegetables into nitrite which when mixed with stomach acid produces NO. If your oral microbiome is imbalanced, it could reduce its ability to produce NO. Certain lifestyle choices can disrupt NO production including use of mouthwash, Fluoride in toothpaste, processed foods & deficient sleep If you think Supplements /Drugs such as Viagra, Ashwagandha & Shilajit could help in curing erectile dysfunction, you are in for the biggest shock. Drugs such as Viagra is not a NO donor but is NO dependent. NO when produced activates a messenger cyclic GMP which is a vasodilator. Viagra makes sure that cGMP does not break down. However if your body is not producing NO, Viagra will never work as the substrate on the basis of which it functions is not activated. Obesity is a global epidemic with more than a quarter of the population dealing with this metabolic syndromeWhile there are tens of thousands of solutions to suppress your weight/make you lose weight, 99% of them are band-aid solutions & do not focus on what makes you obese. Those companies &/or coaches, it is a financial model! You need to maintain your weight, you need to pay them every month to subscribe to their fad diet solutions & working schedules/plans. What causes Obesity? It is low grade chronic inflammation! So you must be wondering how inflammation in your intestines can make you gain weight? There is whole science behind & most of the biological decisions inside our body are driven by our microbiome. You may blame energy metabolism & try to create that energy deficit by eating less & burning more but energy metabolism is not about this mathematical equation. There is a web of molecular features & pathways driven by interaction between your microbiome & external environment & nutrition choices. These microbes impact/produce certain protein /hormones such as GLP-1, PYY Ghrelin & Leptin that impacts your satiety & hunger responses. While these are not the only pathways! Certain microbes can synthesise these drugs into beneficial stuff that have positive correlation with glycemic reduction while some others if active & expressing virulence genes can interfere with synthesis of GLP-1 drugs -Certain microbes if active & expressing virulence genes can degrade GLP-1, making you gain weight or develop type 2 diabetes. In such scenario, these drugs cannot help -It has been found that obese have underexpressed NOS attributed to overexpression of tumour necrosis factor-α caused by microbial derived endotoxin LPS. Besides, there are multiple other factors that can suppress NOS/cause endothelial cell dysfunction such as overexpression of ADMA, oxidative stress( caused by P-cresol &/or Indoxyl Sulphate Production), TMAO & more. Exercise has a vital role to play in fine tuning our physiology & overall health. A moderate exercise with 60-75 minutes per day , 3-5 days a week is important for various biological processes including regulating satiety level, cardiovascular disease, stress response & more. However, an important aspect is exercise performance. Right nutrition choices play a key role in improving skeletal muscle performance. However, in the name of nutrition, there are a number of supplements such as L-Arginine, Creatinine, Beta-alanine & more being sold. However, do we really require supplements? If yes, how much, which supplement? What is the molecular /biological mechanism that makes sure the supplement confers the intended benefit? Can’t such food molecules come from foods? How those molecules are synthesised. On the back of these pertinent questions, most fitness service providers miss the relevance of the most important gas produced inside our body every millisecond- Nitric Oxide. NO is a key molecule for various biological processes including metabolic health, cardiovascular health, sexual health, brain health & more. Without NO, you lose the cardiovascular benefits of exercise. NO is said to improve exercise performance, improve delivery of Oxygen & nutrients to working tissue & more. There are two pathways which stimulates nitric oxide production -Nitric Oxide Synthase(NOS): Endothelial cells line every blood vessel & via NOS facilitates production of NO. NOS converts L-Arginine in presence of oxygen into L-Citrulline & NO. NO is oxidised into nitrate which is stored in skeletal muscle where it gets converted into NO when required & elevates vasodilation effects, thereby improving exercise performance. Consider this, most of these fitness companies would ask you to take L-arginine supplement which is the starting point of disaster. L-Arginine is a semi essential amino acid which our body itself produces. Excess L-Arginine production can lead to overexpression of Arginase Enzyme which competes with NOS for L-Arginine & therefore it is diverted & not put to use for NO production. Instead, it leads to production of superoxide which triggers oxidative stress. Besides, inflammation due to gut microbial dysbiosis can trigger endothelial cell dysfunction & reduces the ability to produce NO. However as we age, ability of our endothelial cells to produce nitric oxide reduces & therefore the second pathway becomes more important -Oral microbiome: Microbes in our mouth that synthesise nitrate in various green vegetables into nitrite which mixes with stomach acid. A portion of nitrite is stored in various tissues including skeleton muscle where it is converted into NO. Rather than just subscribing to supplements, you should understand your biology & functions your oral microbiome perform. Wrong oral hygiene practices such as Mouthwashes can imbalance your oral microbiome & impacts NO producing, thereby directly influencing your exercise performance.

Oral Illness continues to be a huge burden in India with more than 95% of Indians having dental caries & over 50% have periodontal diseases. Most of the current set of dental interventions are band aid solutions resorting to root canals, tooth fillings & more but misses out the most important aspect- the microbes living inside our mouth- ORAL MICROBIOME. One of the important aspect that maintains a healthy oral microbiome is SALIVA The current hygiene practices & oral products, which are meant to upkeep our oral health, are just doing the opposite -Disrupting Oral Microbiome -Impacting oral pH -Interfering with remineralisation One of the common ingredients found in most oral products- Fluoride , assumed to be safe, has been found to be a major culprit. Fluoride is not naturally found in the body & is not biocompatible with our oral biology. If we look back, Decades ago, Fluoride was found to cause less cavities & we have seen dentistry recommending fluoride as a part of oral hygiene. It is not deficient Fluoride but excess Fluoride which is the major culprit. Fluoride is an antiseptic, anti-microbial, neurotoxin & works against our oral microbiome. It has been found that Fluoride is said to impact our brain, IQ, thyroid functions, increase risk of arthritis & more. It has also been found that it is inversely related to melatonin levels & fluorosis is said to reduce our melatonin levels. Besides, Fluoride is electron negative & therefore it is a strong oxidising agent & accepts electrons from other elements. As such Fluoride can combine with other minerals such as sodium ,Phosphate & can enter into every single tissue. In Fact it could interfere with every single cellular process & can even alter our metabolism Fluoride is even said to make oral pH level acidic, creating an opportunity for opportunistic pathogens to grow & thrive. Besides, when consumed in excess amounts, it could replace Hydroxyapatite which is naturally found inside our body and is needed to remineralise our teeth. Sadly most toothpastes & municipal water supply have a lot of Fluoride which is directly impacting not only our oral health but also triggering waves of systemic health issues. 100 million people in India are dealing with type 2 diabetes & counting! India is called Diabetic capital of the world. We have all science & technology & even healthcare to address range of chronic diseases including type 2 diabetes but still the number of Diabetic population seem to increase with current healthcare industry having no clue how to eradicate this epidemic Do you know why? The industry is too much focussed on managing type 2 diabetes through generic nutrition & pharmaceutical approach rather than going deep down to understand & decode hard to detect signals & asymptomatic molecular targets involved in onset & progression of this metabolic syndrome & now we have these new age health tech companies who are promoting diabetes reversal programs!Are these programs really worth it? Do they really reverse diabetes? Answer to these questions is NO . More so as none of these companies address 99% of our biology epigenetically- our microbiome It is important to understand the mechanics behind onset & progression of type 2 diabetes Imbalanced biology triggered by gut /oral microbial dysbiosis creates a situation where first fasting insulin spikes, then fasting glucose spikes & finally Hba1c spikes. The focus has to be to prevent fasting insulin from spiking. If you are suppressing Hba1c or blood glucose levels, the damage has been done. You are just suppressing these plasma level biomarkers. So what regulates insulin response. There are multiple molecules that come into play , produced both by gut & oral microbiome. The most important one that makes sure that glucose is not left in circulation & is transported into your cells for performing various biological processes is NITRIC OXIDE. Nitric oxide is produced via two pathways -Endothelial cells which line every single blood vessel. However our cells ability to produce NO reduces as we age.
