Story (theme) Dr. G won Jen’s book at NHPCO ALC opening key note in Little Rock, Arkansas 10/01/2023 and was most struck by the job description (correlated with anxiety and fatigue of residency’s PGY-1 experience of in hospital on-call life) of family caregivers and their two identity transitions over 4.9 years (average): Life sustaining work to End-of-Life caregiver. Compassion & Empathy by healthcare professionals can save EOL care, perhaps motivate timely Courageous Conversations about dying a good death. Position Description of Family Caregiver: https://mailchi.mp/68c81a8b78db/family-caregiver-position 2. Educate -Teaching point Jen’s late husband would say “You are into PRECIOUS Time” which has a dual meaning for patients and family caregivers: “death is likely, if not imminent” AND “when you say what you need to say and don’t say the things you will later regret” “Precious Time Implementation Guide” is an excellent resource for healthcare professionals who want to be more effective in communicating the reality of the end-of-life situation to patients and their families. EPISODE 33-Breast Cancer Awareness Month 2023: Ever heard about Psycho-Oncology?

Show notes- INTRO Mourning Dove Medical Presents Dr. G at the Heart of Healthcare Guest Dr. Renee Drolet, MD, MA - Founder of Drolet Psychiatry, holistic mental health services. A new outpatient private practice in the Bay Area specializing in psycho-oncology; concierge-level mental health care; medication management and psychotherapy. - Her mission is to expand access in California to patients Mission-Educate & Empower in three parts, helping listeners ‘SEE’ what’s needed to navigate the healthcare system. 1. Story (theme) Former Oncology nurse told Dr. G the work was depressing, and she “felt more like a nurse” once working with hospice. For the patient DISTRESS can occur anytime during a cancer journey: Pre-diagnosis (biopsy, imaging), Treatment (surgery, chemotherapy, radiation), Post-treatment, Remission (new identity), Survivorship (guilt), End-of-Life (existential) 2. Educate -(Teaching point) Dr. Drolet is a psychiATRIST not a psychOLOGIST. Her specialty is psycho-oncology which is collaborative with oncology and palliative care to provide mental healthcare for patients with cancer. Pre-diagnosis can cause initial trauma. Patients can suffer from ANTICIPATORY ANXIETY when a lump is found in the breast or after receiving abnormal mammogram results. The language physicians use can cause “Medical trauma” and PTSD. Battle language has an effect on the psyche of patients. EVERYONE does not respond to treatment or cancer is found at a late stage. When they get to Dr. G they’re often exhausted and frustrated with the healthcare system, many times express feeling abandoned by PCPs and oncologists. EDUCATION of referral sources (ex. hospitals, case managers, fiduciary associations https://pfac-pro.org/ ) and the public about optimal discharge plans to maintain a sense of PURPOSE in the lives of seniors. 2. It is beyond the end of life, consider PRIOR to that: Aging in place at home, or move to RCFEs, home care, mobile physicians, transportation, home health for physical therapy (after a fall and hospitalization), palliative care, hospice. Seniors want to stay at home. Unique models like Parentis Health can catch the time BEFORE seniors go into the hospital, by providing a FULL spectrum of care from very beginning, for stability. This continuum of care or “Umbrella” of services gives seniors 55 & older purpose IN THEIR HOME, prevents crises, mitigates caregiver burnout. 3.
