Follow-up and consistency are the keys. During the first months of any addiction recovery, the firsts months are the most difficult. We all are tempted to falling back. And when you fall down you gotta get back up no matter how bad it hurts. The goal should be to avoid that to happen. In every relationship, is terrifying to think that everything got broken because of our misbehavior, and sometimes their faults. there are always a few stubbornly proud people who consider an impatient word or neglected thank-you fair cause to snub someone for life. Chances are, if your substance abuse problem has lasted any length of time, you’ll finish your list, take a good look at it–and experience a major burst of panic. Remember, you are responsible only for your own actions. The refusal of an apology or reparation doesn’t negate all value in your having offered it; you have done what you could by showing your willingness to accept responsibility. Let it go and consider your duty done. The other party may come around eventually; in any case, you needn’t let the problem continue to weigh you down. Whatever eventually comes of your amends, in being willing to offer them you have taken your first step toward a fresh start. But it means that we admit that are things that fall out of our control. We can turn our will and lives over to the care of God and at the same time take full responsibility for our obligations on our way to healing. It’s worth noting that admitting “I am powerless and my life is unmanageable” is hard to say not only because it threatens to tear from us a habit that’s become a favorite coping strategy and an old friend. The admission strikes a hard blow at our pride, the inherent craving to be capable of perfecting ourselves. Admitting that at this moment we have no strength in ourselves and that we need help is the first step to recovery. Being honest to the point of realizing that we've gone too far and that we must take steps before is too late. Imagining our life getting away from what's been keeping us distant from others, broken our relationships, made us fail in our professional careers. We all make mistakes, sanity is available, and we have what it takes to recover you from addictions. Kemah Palms Recovery is an adult substance abuse facility located southeast of Houston, Tx. In this presentation, the importance of physical distancing but not social distancing is discussed. The doctor goes over opportunities to remain connected while physically separated, including fun ideas for you and your family.

Some of the most common IV drugs used are heroin, methamphetamine, cocaine, and ketamine. In southern and western regions, there is a higher use of stimulants. In the Midwest and Northeast, they use heroin and other opioids primarily. The most commonly injected drugs in Texas are heroin, speedballs (heroin/cocaine), and cocaine. 

IV drug use increases exposure to illness in a variety of ways. Needles & Syringes - Sharing or reusing needles and syringes can spread the Hepatitis C virus. Syringes with detachable needles increase this risk even more because they can retain more blood after being used than syringes with fixed needles. Preparation Equipment - Equipment, such as cookers, cottons, water, ties, and alcohol swabs, can quickly become contaminated during the drug preparation process. Fingers - Contact with infected blood can spread Hepatitis. Blood on fingers and hands can contaminate the injection site, cottons, cookers, ties, and swabs. And the last is Surfaces - Spreads when blood from an infected person contaminates a surface, and then that surface is reused by another person to prepare injection equipment.

With HIV and IV Drug Use, 1/10 new cases are attributed to injection drug use or M2M unprotected sex and injection drug use. Many don’t realize that HIV can survive in a used syringe for up to 42 days, depending on temperature and other factors. There is a 1/160 chance of transmitting HIV every time a person injects drugs.

When sharing needles, you risk contracting or transmitting HIV. According to the Centers for Disease Control and Prevention, injection drug use is the second leading cause of HIV infection for African American women and the third leading cause for African American men. Approximately 1 in 10 or 10% of HIV diagnoses among Hispanics/Latinos were attributed to injection drug use, including 3% of cases among MSM and IDU. White female IV drug users have the highest rate of HIV at 50%.

With HIV, many times, there are co-infections like syphilis, gonorrhea, and herpes. There are ways to protect yourself if you have an HIV-positive partner. Infection is less likely if the person with HIV is on a treatment called antiretroviral therapy (ART). ART can produce an undetectable viral load with virtually no risk to HIV-negative partner. They also encourage them to choose less risky sex activities and use a new condom for every act. Risk is also reduced with HIV-negative partners taking PreP consistently; this only works to prevent infection of HIV.

How to treat someone with HIV and Substance Use Disorder. 

• Treat both concurrently

• Assessment /referral for SUD/MH treatment – most appropriate level  of care to support HIV diagnosis

• Harm reduction/MAT/SSP’s (syringe services programs)

• 6 in Texas –Dallas, Houston, Waco, Austin, Ft. Worth, Abilene

• PEP – Post-exposure prophylaxis

• Medication after exposure within 72 hours. Must take for 28 days. Not ongoing, emergency use only.

• PrEP – Pre-exposure prophylaxis

• Not positive, but high risk. Blocks the HIV virus from entering the cell. One pill daily for 30 days. Strict adherence for the best outcome. Not guaranteed

Why motivation, you might ask? Here are a few essential components to consider. Motivation is key to change; motivation can be changed. Motivation is evidence-based and relationship-focused. Motivation access existing strengths and has an emphasis on self-determination. 

The Transtheoretical Model…

The Stages of Change Model focuses on decision making and intentional change. People do not change behaviors quickly and change in behavior occurs continuously through a cyclical process. Different behavioral theories and constructs can be applied to various stages of the model where they may be most effective. The Transtheoretical Model is based on more than two decades of research; individuals move through a series of stages in the adoption of healthy behaviors or cessation of unhealthy ones. (Prochaska & Velicer, 1997). This presentation then reviews the Stages of Change Model. 

Assessing Readiness for Change Cont'd...

The clients need motivational support appropriate to their stage of change. Inappropriate interventions could cause additional treatment resistance. The clinician must accept the client's current position and avoid "getting too far ahead" of the client. Apply the correct counseling strategy for each stage of readiness. A great tool to assess the readiness to change is the university of Rhode Island Change Assessment Scale (URICA). This is used at the start of treatment to measure the state of change: pre-contemplation, contemplation, action, and maintenance. Clinicians may use the URICA to evaluate an individual's level of motivation for change and use this information to help guide treatment approaches. • Assess stage of change over the course of treatment

• Identify barriers to recovery and engagement 

• Develop and reinforce motivation for change

• Assist client in working toward self-identified goals

• Assist client with an accurate understanding of the problems and risks associated with their substance use

What is Motivational Interviewing? Motivational Interviewing (MI) is a collaborative, person-centered form of guiding to elicit and strengthen motivation for change. It is a way of working with people to assist them in accessing their intrinsic motivation to change behaviors that contradict their essential values and interfere with achieving their life goals. MI is both a philosophy and a set of strategic techniques. Motivational Interviewing aims to produce internally motivated change, mobilize the client's own change resources and move from developing motivation to strengthening motivation for change. 

The goals of Motivational Interviewing are to resolve ambivalence, avoid strengthening resistance, elicit self-motivational statements, enhance motivation and commitment to change, and assist the client in progressing through the stages of change.
