Dr. Morgentaler, is a leading international figure in the fields of testosterone therapy, prostate cancer, and male sexuality. He graduated from Harvard College in 1978, and Harvard Medical School in 1982.

Dr. Morgentaler is credited with shattering the decades-old belief that testosterone therapy is risky for prostate cancer, and pioneering the modern use of testosterone in men. More recently, Dr. Morgentaler has been a leading scientific figure in the global discussion regarding cardiovascular risks of testosterone therapy.

Dr. Khera lives in Houston, TX practices at Baylor and has dedicated his clinical and research efforts to three main areas: Men’s Health, sexual medicine, and hormone replacement therapy. His basic science and clinic experiences have allowed him to thus far give over 250 lectures at scientific meetings throughout the world, publish over 120 articles in peer-reviewed journals, complete 15 book chapters, and edit and write two books, all in the

Dr. Khera was part of the NEJM study that came out this summer looking at T in men with low T and high risk CV risk. The Traverse
trial. Conclusions: In men with hypogonadism and preexisting or a high risk of cardiovascular disease, testosterone-replacement therapy was non-inferior to placebo with respect to the incidence of major adverse cardiac events.
Barriers to a testosterone prescription include that it is a regulated medication, a physician needs a DEA license to prescribe it – similar to a narcotic. Dr. Khera lends his opinion to safe prescribing of testosterone but thinks the DEA regulation is here to stay.
He clarifies the number of men who suffer from hypogonadism and the number of men who are undergoing treatment for this
condition?
Testosterone levels are dropping because men are getting less healthy. Dr. Khera talks about measuring a baseline
testosterone level in men as a marker of overall health and focus on lifestyle changes to naturally get this normal.
50% of men don’t talk to their doctor about their
erectile dysfunction, and 44% of those men don’t talk to their partner about
it.

Dr. Lauren Fogel Mersy (she/her) is a licensed psychologist and an AASECT certified sex therapist. She received her doctorate in clinical psychology and completed her postdoctoral training at the University of Minnesota’s Institute for Sexual and Gender Health. She has advanced training in Emotionally Focused Couple Therapy and Gottman Method Couple Therapy. She specializes in sexual health and relationships and owns her own private practice in Minnesota.

"Your desire should match your partner" is a myth.

Why do we want to blame someone or make someone be the problem in a relationship for having a low libido.

The stereotype is that the low libido person is the female – but that is not always the case.

We define responsive desire.

Four requirements for responsive desire – consent, pleasure, focus, time

When you have sex for your partner or fulfill a duty – if this is the default or only motivator for sex it can really affect your desire. Tips for getting out of this pattern.

We talk about shame and sex.

Your sexuality does not exist in a bubble.

We suss out asexuality versus low desire. Asexuality is a self-identity label and a valid sexual orientation. What to do when not attracted to a partner anymore?

Is attraction as fluid as desire is?

We talk about “sexual destiny” beliefs versus sexual growth beliefs and how these can hurt and benefit our relationships.

 

Tips for the high libido partner. Reducing pressure on their partner and managing their rejection and disappointment reactions. Consider what you are longing for when you seek sexual connection or sexual frequency.

Menopause and Hormones in the Modern Era AKA: Why the Boomers Should be Pissed

 This is a lecture I gave for the Society of Women in Urology about hormones.

 Genitourinary syndrome of menopause

 Men at age 60 have more estrogen in their bodies then women

Why I don’t do a loading dose of vaginal estrogen.

 The problem with the vaginal estrogen labeling by the FDA. 

How the nation used estrogen prior to the Women’s Health Initiative Scare.

I explain why the WHI study was created and what question it was trying to answer.

Myth: you have to be a year without a period to be on hormones.

 I break down the three concerns about progestin and breast cancer from the WHI

1)  Was the placebo arm flawed

2) Was it the synthetic progestin and now we use bioidentical

3)  Maybe progestin increases risk but it is small and perhaps benefits still outweigh risk as breast cancer is so curable and all the other benefits are worth the small increased risk

I discuss the “critical window” hypothesis and “healthy cell” hypothesis and why hormones are recommended to be started “early” in menopause which means less than ten years. Why I think the Boomers should be pissed.

Vaginal estrogen for all and get an okay from your

 Best practices

-  If you have a uterus, with systemic estrogen you need a progestin

-  You can continue hormones for life as long as your benefit > risks

-  Vaginal bleeding after menopause is never normal

-  If there is an FDA approved product use it instead of compounded

-  Checking hormones is often not necessary

-  One size does not fit all

 

What about hormones for prevention? On a national level the answer is no. But everyone needs their own analysis.

 National guidelines on how to give testosterone to women for low desire.

Hormones: are we being equitable to all genders in regards to hormones.

 

NAMS 2022 guidelines: According to NAMS, “the benefits of hormone therapy outweigh the risks for most healthy symptomatic women who are aged younger than 60 years and within 10 years of menopause onset.” For women with primary ovarian insufficiency and premature or early menopause who are at higher risk of bone loss, heart disease, and cognitive or affective disorders, “hormone therapy can be used until at least the mean age of menopause unless there is a contraindication to its use.”

Estrogen Matters with Dr. Avrum Bluming
Avrum Bluming received his MD from the Columbia College of
Physicians and Surgeons. He spent four years as a senior investigator for the National Cancer Institute.
He organized the first study of lumpectomy for the treatment
of breast cancer in Southern California in 1978, and for more than two decades he has been studying the benefits and risks of hormone replacement therapy administered to women with a history of breast cancer. Dr. Bluming has served as a clinical professor of medicine at USC and is the co-author of “Estrogen Matters: why taking hormones in menopause can improve women’s wellbeing and lengthen their lives – without raising the risk of breast cancer” with Dr. Carol Tavris.

Hear his story about how he became the advocate for hormones and hormones after breast cancer.

So many people think correlation = causation – case with E and breast cancer – just poor
education in our population?

“we don’t know what causes breast cancer” – can people handle a grey world?

Estrogen receptor positive doesn’t mean estrogen caused the cancer – discuss

Dispel the myth that early puberty and late menopause is a breast cancer risk – dogma – “lifetime exposure” – but women with lots of pregnancies have lower risk.

How common is pain with sex for women?

Often times with pain with sex there is a physical cause, but you can’t always see it.

Pelvic pain and pain with sex is very isolating and people feel very alone.  People don’t know how to talk about it. 

What happens after the pain is fixed.

What does “all in your head” mean and why people don’t like that.

“Those who perceive their partners as kind and understanding report more intense pain than women whose partners have responses that promote adaptive coping.” – what!  What is adaptive coping?

How to communicate your pain to your partner and your doctor.

What if you are a partner of someone with dyspareunia - - “best partner response is having a partner who engages in what researches call facilitative responses – affection and encouragement to cope with the pain in an adaptive way, such as by focusing on pleasure, deep breathing or positive thoughts while experiencing the pain.”

We talk about stress, sex and the gut – things like irritable bowel syndrome related to pain with sex?

The role of birth control pills and pelvic pain and health. In her book, “When Sex Hurts” they talk about Yaz (drospirenone) and pelvic pain – a prospective study of women’s genitals and sexual function prior to starting Yasmin – and then after three months- decreased clitoris, decreased blood flow, decreased labia minora, decreased libido, fewer orgasms and more dyspareunia.

The role of testosterone in pelvic pain – one study showing women with higher androgen levels have less work related neck and shoulder pain then women with low androgen and differing levels of Testosterone may explain some of the gender related differences in pain perception

The difficulty in giving testosterone to women and the problem with pellets.

-       "Twenty-five studies of HRT after a breast cancer diagnosis, published between 1980 and 2013, are discussed, as are the 20 reviews of those studies published between 1994 and 2021. Only 1 of the 25 studies, the HABITS trial, demonstrated an increased risk of recurrence, which was limited to local or contralateral, and not distant, recurrence. None of the studies, including HABITS, reported increased breast cancer mortality associated with HRT. Even in the HABITS trial, the absolute increase in the number of women who had a recurrence (localized only) associated with HRT administration was 22. It is on the basis of these 22 patients that HRT, with its demonstrated benefits for so many aspects of women's health, is being denied to millions of breast cancer survivors around the world."

Feeling Broken In The Bedroom? What You Need To Know About Your Sexual Health - With Dr. Kelly Casperson

This is an interview I did with Lesley McShane for her podcast “Redesigning Midlife” and it was just such a good interview I asked her if I could share it again. 

1. Basics:  a. Menopause means it’s been one year and one day since your last period. b. The average age of menopause is around 51. and c. for ten years prior to menopause, women go through peri-menopause, where hormones are doing a reverse puberty.

2.  A lot of women stop or have pain during penetrative intercourse during this time due to a decrease in natural lubricant and collagen, but this can be easily treated.

3. Even if you don't have menopause symptoms, if you are over the age of approximately 51, you have gone through it.

4. The most sexually satisfied people aren’t young people, it is people in their 50s, 60s, 70s.

5. Great sex is not a passive thing. It’s an active process, just like someone that is physically fit. It has to be prioritized.

6. Use of lube increases your success of orgasm by 70-80%. Silicone based lube is best because you don’t absorb it like you do the water based.

7. There is no “how long” should I use Vaginal Estrogen cream. The use of it is like using sunscreen or wearing a seat belt.

8. You don’t need a “sex drive”. We do things because they are awesome.  If you aren’t having awesome sex, don’t wonder why you don’t desire it.

9. We feel entitled to a spontaneous sex drive that has been sold to us by media. And when we don’t achieve that, we end up feeling broken.

10. Putting something in the vagina only leads to female orgasm 30% of the time so special focus need to be paid to the female body. Your largest sex organ is your brain.

11. Don’t sit passively by waiting for spontaneous desire to happen. Get dressed up and go to the party even if you don’t think you want to. Chances are you’ll have a good time once you get there.

12. Control of stress and cortisol levels in our bodies will help us desire more intimacy.

13. If there is a desire mis-match in your relationship, normalize it by talking about it.

14. Communication with our partner about menopausal symptoms and what we are going through is important to our relationship.

15. Get educated on hormone replacement therapy as an option. When women start thinking about how they want to live as they get older and get educated, then they see how beneficial HRT could be for them.

16. Women between the age of 50-60 that are on hormone therapy live 3-4 years longer than women in the same age group not on hormone therapy.

17. For our young adult children that are starting their own sexual lives, the biggest thing they need to know from the start is about consent.

18. They need to be aware of is orgasmic equality.

19.We have to educate our girls that pleasure is for everybody.

Conclusions: These results suggest that DHEA treatment may have significant antidepressant effects in some patients with major depression. Further, larger-scale trials are warranted.
