Today we will be joined again by Dr Sreedharan Gastroenterologist from ULHT to discuss the new pathway and flowcharts created to improve identification and management of patients with a hepatitis presentation on their liver biochemistry (predominantly raised ALT). 

All the new pathways are incorporated into your clinical systems and we encourage you to play around with them, reading the explanatory notes and using the flowcharts with your patients.

We will also summarise some of the key points from the liver pathways series.

Welcome back to the penultimate episode in our liver pathways series on abnormal liver function tests. Today we will be joined again by Dr Sreedharan to discuss the new pathway and flowcharts created to improve identification and management of patients with a cholestasis presentation on their liver biochemistry - predominantly raised alkaline phosphatase (ALP) which may occur with a raised gamma GT (GGT) as well.

It is a really exciting time for the Gastroenterology team who have been working with primary care clinicians to update the liver pathways and streamline care for patients. There are changes to the open access endoscopy service to be discussed plus the effect of the pandemic on referral waiting times.

Episode 17: Rheumatology Referral Forms & Pathways

We are joined today by Dr Batsi Chikura Consultant in Rheumatology at United Lincolnshire Hospitals. This episode we discuss changes to the Rheumatology service. There are now a number of new referral forms in place which can be accessed within your practice clinical systems - EMIS or System One. You will find the new referral forms in exactly the same place you are used to finding referral forms within your clinical system. Please use them and share them with your colleagues.

New referral pathways are now in place for patients with Ankylosing Spondylitis, Early Inflammatory Arthritis, Polymyalgia Rheumatica, Gout, Connective Tissue Diseases and Giant Cell Arteritis. There is also updated referral guidance being developed for patients with Fibromyalgia and Ehler's Danlos Syndrome. The service has been changed to streamline and improve the care of these patients. There is also much increased use of the Advice & Guidance route so GPs can get timely advice from specialists regarding their patients.

This service is going through a period of review so we discuss what that means, the current service and expected changes. As you may be aware Lincolnshire has launched a new Dementia pathway so we are really keen to open up the conversation around life with dementia. It is in fact World Alzheimer's Day is on the 21st of September. The focus this year is on 'Journey through the diagnosis of Dementia'. The national diagnosis rate has dropped to 62.1% over the last 18 months; internationally, 90% of cases are undiagnosed - in Lincolnshire the aim is to change this through active working through the new pathway.

One of the challenges for people and families affected by dementia is getting a diagnosis in the first place. They struggle because they feel they aren't being listened to or having concerns dismissed. Diagnosis isn't just about access to medication. It is about recognition and giving people the support they desperately need to deal with the challenges they face.
