HAPPY NEW YEAR #ThePaedipodsCast Listeners! Thank you for all your ongoing support as we enter our third year of providing you with interviews with leaders for the field of global paediatric orthopaedic surgery. 

In this episode, podcast host Mr Pranai Buddhdev (@Pranai_B) visited the newly opened CURE Hospital in Bulawayo, Zimbabwe and had the opportunity to sit with their senior lead paediatric orthopaedic surgeon & Chief Medical Officer, Dr Rick Gardener. Heralding from the UK, training in Bristol and subsequently at Sick Kids Toronto, he embarked on a consultant career in the African Sub-continent following experiences that took him to Malawai & South Africa during training. He commenced his post at the CURE Hospital in Ethiopia for 7 years, including opening up the first Gait Lab outside of South Africa and being acknowledged by the Ministry of Health of Ethiopia for his services. He subsequently has moved to Bulawayo in Zimbabwe, where a new hospital was opened by the President in May 2021. Delivering excellent paediatric orthopaedic care is at the heart of the mission of CURE international, which spans across 8 countries and served over 5 million patients to date.

WELCOME BACK to #ThePaedipodsCast! It was so great to meet so many listeners IRL/F2F at the BOA Meeting in Aberdeen in September, and to get your feedback and suggestions for future guests. 

This Months guest is a complete 'all-rounder-big-hitter' in paediatric orthopaedics. Mr Martin Gargan heralds from York, trained at Oxford and was inspired into a career in paediatric orthopaedics early on his career. Having spent some time at Great Ormond Street and on a trauma fellowship in DEtroit, MI, he was appointed as a paediatric orthopaedic consultant in Bristol in 1994. He developed an incredible broad practice with a focus on hip preservation, introducing the PAO procedure to the region under the tutelage of David Reynolds. 

He has accomplished a huge amount including as TPD for the SW Deanery where his trainees had a 100% pass rate on the FRCS, and even got to select their own rotations to maximise their training opportunities. Serving as the chief examiner of the FRCS has given him an insight into how to best train the next generation of surgeons, and serving on the BOA Medicolegal committee as a member/Chairman has given the experience to better understand how we can deliver safe care through teams and continual review and improvement. 

Most notably, he was appointed as the Harold & Bernice Graves Chair of Orthopaedic Surgery at the Hospital for Sick Children in Toronto from 2014-18 where he developed incredible leadership and management experience which has led to him being appointed as the Divisional Chair of Women's & Childrens at University Hospital of Bristol group.

He has championed collaboration globally to develop treatment pathways for paediatric orthopaedic conditions including Clubfoot, DDH Screening and CP Hip Surveillance - a true feat once you consider it involves the education & training of over 120,000 medical professionals. We talk about his research endeavours, his adoption of telemedicine and virtual education and his journey into introducing such large changes in a complex and fragmented healthcare system.

He was a committed and excellent surgeon, with specialist training undertaken in Canada, Australia and USA. He has a masters in epidemiology, and we talk all about the importance of addressing the prevention of injuries, with trauma being the no.1 cause for children globally. We also talk about the role of global surgery, the Bethune round table and his thoughts on how to develop and lead a world-renowned department in his role at SickKids.

He is the Chief Technology officer and Founder of  the Digital Orthopaedics Conference SF (DOCSF) which is held each year in January where startup companies meet each year to discuss and share ideas as to how technology can disrupt th orthopaedic industry, with a vision to create a movement that delivers digital health (lower cost, higher quality, and better access) by focusing on technology within musculoskeletal care.

Professor Michael Millis is a world-renowned paediatric orthopaedic surgeon with a specialist interest in hip preservation for over four decades. He was the founder of the hip preservation program at Boston Childrens Hospital after spending time with Prof Heinz Wagner in Germany as part of an AO fellowship in his early career and has pioneered innovative techniques in North America. He has been a driving force in research and education as a Professor of Orthopaedic Surgery at Harvard Medical School and has been recognised by EPOS, receiving  the Pro Maximis Meritis Award from EPOS 2017, by POSNA as the Presidential Guest Speaker in 2018 and by the Bern Hip Symposium as the Guest of Honour in 2020. In  this episode, we explore the journey of his career,  how he set up his specialist practice, and his concepts  of 'hipology & happology'.

As we enter our third month of the UK lockdown, we have had very little developments in our understanding of how the Coronavirus Pandemic will be defeated and how we can recover in the healthcare sector. With the hope to avoid a second peak of COVID patients, elective surgery was halted initially, with the hope to resume by mid-Summer. There has been some guidance from the BOA and NHS England with regards returning to some normality, however questions remain with regards to the safety of elective surgery, recovery, face-to-face consultations and its undenied impact on the healthcare workforce that have been pushed to their limits over the last few months. Orthopaedic surgeons from all sub-specialties are amending the way they work in preparation of potential redeployment due to the #COVID19 Pandemic. Norfolk & Norwich hospitals have run paediatric virtual fracture clinics for over 2 years, and this discussion with Mr Anish Sanghrajka, we explore how we can reduce foot fall in hospitals significantly whilst maintaining patient safety & standards. 

The BOA has promptly released modified BOAST guidelines and BSCOS has updated members with suggestions to modify practice. We will discuss these in a further episode in the upcoming days. Please find resources below:

Dr. Narayanan’s clinical practice has a major focus on the care of children with CP in addition to an interest in the management of congenital, and acquired lower limb deformities and length discrepancies and paediatric trauma.. His research program has focused on understanding patient priorities and their role in clinical decision making, and for the development and validation of meaningful outcome measures, and applying these in clinical trials to evaluate the effectiveness of interventions and health technologies for pediatric musculoskeletal disorders particularly CP.
