Gretchen Frey, MD – hormone therapy, gender-affirming care, sexual pain disorders
This episode is with OBGYN Dr. Gretchen Frey. She has a new project out now! An Introduction to Sexuality Education: A Handbook for Primary Care Providers. Get it now at v3.unhushed.org/
She talks about how she worked as a general OBGYN for many years, then as she transitioned away from OB and focused on clinic GYN she saw a lot of patients in menopause and peri-menopause. She became comfortable using hormone replacement therapy in that context and then expanded her practice to incorporate hormone therapy in gender-affirming care as well (which she says there is a great need for this type of care in primary care, particularly in Denver where I am at). She discusses sexual pain disorders such as vulvodynia, dyspareunia, and vaginismus, and why they can be difficult to treat. She notes that having a multi-specialty team is greatly important for success in this.

She talks about how she got out of the world of corporate medicine to start her own small, independent practice that functions as direct primary care (DPC), circumventing insurance and giving her patients quality time and quality care. She talks about the details regarding DPC and how it functions. We hit on other topics such as osteopathy in general, and how she uses OMT. Today's episode is with Marcia Brauchler, a business consultant for physicians. She is the owner and founder of Physicians' Ally, a consulting firm that helps physicians negotiate contracts with insurance companies so they can realize their maximum worth and value as providers. We talk extensively about contract negotiation, coding and billing, and compliance. Believe me, this is an exciting topic because so little of what she does in her world is taught to physicians anywhere in their training process. She loves to work with doctors of all kinds to help them not only get reimbursed what they are worth, but in doing so, also deliver better care to their patients. In 2020, Marcia launched a partnership with the University of Denver to teach her CPC Exam Prep Coding Class.
