Episode 56: Sexual Behavior and Dementia
In this episode, I want to address a topic that can be uncomfortable: sexual behavior in people living with dementia. Sometimes, an aging parent with dementia may make overtures to an adult child of the opposite gender. I explain how this is a case of "mistaken identity" and how to prevent it. There are times when the person living with dementia wants sexual activity, but the spouse, for a variety of reasons, cannot connect in that way. Episode 55 FTD and Primary Progressive Aphasias
In today’s episode, I describe two subtypes of frontotemporal dementia: nonfluent variant primary progressive aphasia and semantic variant primary progressive aphasias. Unlike behavioral variant FTD, people with either one of the primary progressive aphasias have problems speaking as the initial symptom. Episode 47: Being Your Authentic Self by Using the "Entering Their Reality" Approach to Handle Dementia Behaviors
"Don't piss on my head and tell me it's rain." I don't like being lied to, even if you think the lie is sparing my feelings. I used to lie to people living with dementia because I thought it was kind...until I got caught in a lie. I also felt very fake. Feeling fake can mess up your vibes and increase the behaviors. In this episode, I dive deeper into one of my favorite strategies, "Entering Their Reality." I use this approach when I'm faced with care refusals and nothing else works. Entering Their Reality is not straight up lying. This approach takes into account the person's type of dementia, their personality, their cohort, and their values. When you "enter their reality," you respond in a way that is aligned with A TRUTH and taps into their values. If this sounds like some weird philosophical approach--it isn't. Listen in and I'll walk you through the strategy, and give you some concrete examples at the end. This podcast was extracted from a live webinar I gave Monday, May 30 2022. Episode 41: Three Ways to Know if it is Safe to Leave My Family Member with Dementia Alone
“Is it safe for me to leave my loved one with dementia alone?” This question popped up as I was working on a piece about caregiver self-care. Dementia caregivers constantly ask me “How do I take time for myself if I’m ‘on duty’ or ‘on call’ 24/7?” Most dementia caregivers know they need to take care of themselves, but they find themselves last on their lists because the needs of their loved ones seem greater than their needs. There are also times where the caregiver has to go somewhere, like a doctor’s appointment or to the hospital for a test, and taking the person living with dementia is not an option…and the person living with dementia is insistent that they can be left alone. Yes, you want to respect their wants and autonomy. The last thing you want to do is to put in place unnecessary restrictions. But how do you know if leaving them alone is safe? Episode 40: Deciding to Place a Loved One with Dementia is Not Surrendering!!
There is this belief that "good" caregivers keep their loved one living with dementia at home and "bad" caregivers move their loved one into care--into assisted living facilities, memory care units, nursing homes, etc.  This belief is f*cking bullsh*t. There are many reasons why home care may not be the safest situation, both for the caregiver and the person living with dementia. I talk about the obvious ones--like caregiver physical and mental health and unsafe behaviors. Episode 32: What is Sun Downing and How to Handle It!
Sun downing--a period of seemingly increased confusion and agitation that may occur in the afternoon/evening but can occur at any time--is a behavioral symptom of distress. Sun downing, like many dementia-related behaviors, is treated like "just one of those things." Like an inevitable behavior that caregivers just have to put up with. I disagree. Having dementia does not alter physical, emotional, or social needs. Instead, the brain changes from dementia make it tougher for people to have those needs met. Shadowing is a very challenging behavior faced by dementia caregivers. The person living with dementia has to be physically near the caregiver. Episode 22: Should My Family Member with Dementia Be Driving?
I often hear, “It’s ok for my family member to drive, even though he has dementia (or Alzheimer’s). He doesn’t go far. He only goes to local places, like Walmart and church.” Listen as I offer some questions for you to honestly ask yourself about their driving:  1. Is the person with dementia getting lost in familiar places?  A "yes" response to this question is a definite NO, the person should not be driving. 2. Any recent accidents or "near-misses"?  3. Any new damage showing up on the car? 4. Any impulsive or road-rage behaviors? 5. Sudden pattern of traffic violations? 6. Would you feel comfortable riding as a passenger if the person with dementia were driving? 7. Would you feel comfortable having your child or grandchild as a passenger if the person with dementia were driving? Episode 20: How Do I Know if My Family Member with Dementia Can Safely Make Decisions?
In today’s podcast, I answer the question, “How do I know if my family member living with dementia can safely make decisions?” This concern usually comes up around financial issues and living arrangements. It is important for caregivers to understand that the ability to make a decision is not an "on/off" light switch. It is more like a gradual dimmer and must be evaluated for each individual and for each circumstance. The ability to make a decision is called decisional capacity. How do you know if someone living with dementia has the decisional capacity for a specific problem or situation? These 5 questions can start the conversation rolling. Caregivers will probably have other related questions for each of the big ones. Tell me about the decision. (What is the decision? What does it mean to you?) Episode 19: How to Handle Hallucinations in People Living with Dementia
Hallucinations refer to sights, sounds, feelings, and smells that come from inside the brain. Hallucinations are common with Lewy Body Dementia but can happen with any dementia. The person with dementia is seeing, hearing, or smelling stuff that the rest of us are not experiencing. Hallucinations can also happen when people living with dementia experience delirium. Delirium occurs when people living with dementia suddenly become more confused than usual; infections, especially bladder infections, can cause delirium in people with dementia. In people with dementia, hallucinations may arise from changes in the brain and the brain chemicals. Hallucinations may be scary, like seeing a stranger standing next to the bed. Or they can be comforting, like seeing and hearing young children playing in the living room. Sometimes the hallucinations may be more like vivid memories, such as when a person with dementia sees a long-dead family member from long ago. Care refusal is a real pain in the ass. It makes caregiving that much harder. Learn how to hack procedural memories and avoid triggering care refusals using your own appearance and the environment. Other techniques include short 1-step commands, gestures/pantomimes, encouraging self-care (and how to actually do this!), schedules and routines, avoiding elderspeak, chaining, vibes, dementia time travel, and entering their reality. I developed and tested these strategies in my own research and have published this information in boring nursing/medical journals that no one ever reads. Episode 15: Tony Bennett, Salmon Patties, and Fridge Worms: How to Use Meaningful Activities to Maintain Function in Persons with Dementia
Well-meaning family and formal caregivers sometimes start to "overdo" for persons living with dementia. "I don't want her to get hurt" or "I don't want him to feel embarrassed" or "It is just so much easier if I do (fill in the blank)." What people do not realize is that once a person with dementia forgets how to do an activity or task, he or she is rarely able to relearn it. There just aren't enough nerve cells and brain chemicals to knit together a new memory. People with dementia are vibrant, wonderful people...who happen to have a little, or maybe a lot, of forgetfulness. Here's the thing...the drive, the search for meaning, the desire for affection, the desire to be HEARD...remain. What changes is that the OUTLETS for the expression of these important desires and drives get cut off by the dementia...regardless of the type. The loss of these outlets create anger, frustration, irritability and other behaviors. Sensitive and meaningful interactions with others can reduce the isolation felt by people with dementia. WTF is dementia with Lewy Bodies? Is Alzheimer's the same as dementia? Learn a bit about the 5 most common types of dementia: Alzeheimer's dementia, vascular dementia, dementia with Lewy Bodies, Parkinson's disease dementia, and fronto-temporal lobar degeneration (aka fontal temporal dementia).
