His family is very brave sharing this information.

Episode 68: Using the "Caregiver Vibes" Strategy for Care Refusals

It is really frustrating when the person living with dementia refuses to accept care or tells you that he or she already performed the task. You know the drill. “Take a shower,” you say. “I just did,” they say…even though you know it has been days, weeks, maybe months since they have had a shower. Bathing isn’t the only challenging behavior. Convincing a person with dementia to remove soiled clothing can be a nightmare, a literal tug-of-war between the two of you.

One strategy that is useful is called “caregiver vibes.” This strategy has two parts. The first part is to become calm. Yes, I know that can be a tall order when you are exhausted and overwhelmed. I recommend taking 3 deep breaths. Seriously—breathing long and deep helps to reset the vagus nerve and slows down the heart rate. The second part is to picture yourself successfully getting the care done.  In this podcast episode, I explain in detail how to accomplish both steps. I also explain how to integrate "caregiver vibes" with other strategies to gain cooperation.

Weight loss in people living with dementia can stem from a variety of causes: the need for more help or supervision; being too sedated to properly eat; pacing and unable to sit still to consume a meal; or refusing to eat. In this episode, I provide approaches for all of these causes and provide strategies if your family member is refusing to eat.

Apathy has different causes in people living with bvFTD than people with other dementias. Why does my family member’s dementia look differently from others with the same diagnosis, or even at the same stages?

I get this question multiple times a week. Sometimes, caregivers are confused because this is the second, or third, person they are caring for…and their experience this time is very different from earlier experiences. Or, a caregiver is going to a support group and realizes that their loved one’s memory problems are not consistent with what other members are describing. Caregivers start to wonder if the person living with dementia is “pretending” to have memory problems or if the diagnosis is correct. Some caregivers may even believe that their family member could do more if only they would try harder.

One reason why two people with the same diagnosis may behave differently is because of something called “cognitive reserve.” Cognitive reserve is the build up of extra neurons in different parts of the brain. Education, hobbies, and occupation can all build cognitive reserve.

Episode 62: A Variation of Alzheimers--Logopenic Primary Progressive Aphasia

In episode 55, “Frontotemporal Dementia and Speech Problems,” I introduced something known as primary progressive aphasias. Primary, because the speech problems happen first in the disease. “Progressive,” because the speech problems get worse over time. “Aphasia” is a medical word for difficulty speaking. Primary progressive aphasias affect 200,000 people in the United States and are considered “rare.” There is nothing rare about a disease when it affects you or a loved one. PPAs are associated with dementia; instead of memory problems happening first, speech problems happen first. Memory problems happen later. It is the speech problem that gets people to come to a neurology practice, where the specific speech problem is identified.

Capgras syndrome is the delusion (false belief) that one's family have been replaced by duplicates...by imposters. Capgras syndrome is seen most frequently in people living with Lewy Body dementia but can also occur in other dementias. The breakdown of specific parts of the temporal lobes, occipital lobes, and limbic system connections are thought to be the culprits. “Is it safe for me to leave my loved one with dementia alone?”

This question popped up as I was working on a piece about caregiver self-care. Dementia caregivers constantly ask me “How do I take time for myself if I’m ‘on duty’ or ‘on call’ 24/7?” Most dementia caregivers know they need to take care of themselves, but they find themselves last on their lists because the needs of their loved ones seem greater than their needs. There are also times where the caregiver has to go somewhere, like a doctor’s appointment or to the hospital for a test, and taking the person living with dementia is not an option…and the person living with dementia is insistent that they can be left alone. Yes, you want to respect their wants and autonomy. The last thing you want to do is to put in place unnecessary restrictions. But how do you know if leaving them alone is safe?

There is this belief that "good" caregivers keep their loved one living with dementia at home and "bad" caregivers move their loved one into care--into assisted living facilities, memory care units, nursing homes, etc. 

This belief is f*cking bullsh*t.

There are many reasons why home care may not be the safest situation, both for the caregiver and the person living with dementia. I talk about the obvious ones--like caregiver physical and mental health and unsafe behaviors. I also talk about topics that are rarely discussed, like caring for a toxic relative or  having zero aptitude for dementia caregiving. I also talk about ways to release caregiver guilt about placement. I want family caregivers to know that placement does not mean you failed. It does not mean you are a rotten son, daughter, partner, spouse, sibling or friend. It means that you are continuing to care for the person with dementia. You are looking for the best option for ALL concerned.

Sun downing--a period of seemingly increased confusion and agitation that may occur in the afternoon/evening but can occur at any time--is a behavioral symptom of distress. Sun downing, like many dementia-related behaviors, is treated like "just one of those things." Like an inevitable behavior that caregivers just have to put up with. I disagree. Having dementia does not alter physical, emotional, or social needs. Instead, the brain changes from dementia make it tougher for people to have those needs met. Shadowing is a very challenging behavior faced by dementia caregivers. The person living with dementia has to be physically near the caregiver. Caregivers will tell you that they cannot even go to the bathroom without their loved one banging on the door or pacing right outside. Certified registered nurse practitioner Lindsey A. Robbins explains the behavior and how caregivers can best manage it. Ms. Robbins has worked with people living with dementia in long-term care and in home-health. She currently works in a neurology clinic that specializes in dementia care.
