Dr. Abramowitz takes a deep dive into some of the major flaws of this classification system, including the lack of an underlying theory and issues with reliability and validity. Multiple examples of the current issues with the DSM are applied to clinical situations, and Dr. Abramowitz begins to discuss solutions to these problems. Dr. Waltman and Dr. Duncan discuss socratic questioning, its origin with the Greek philosopher Socrates, and how it can be an effective tool in therapy. They provide numerous benefits to this intervention, including helping clients “become their own therapists.” Dr. Waltman discloses the framework for honing the skill of socratic questioning from his new book, Socratic Questioning for Therapists and Counselors: Learn How to Think and Intervene Like a Cognitive Behavior Therapist. Dr. Waltman also identifies a number of other pitfalls therapists make when implementing CBT, such as setting an agenda, pacing the session, receiving feedback from clients, reviewing and developing individualized homework, trouble selecting when to implement interventions and deciding where to intervene. He describes his own therapeutic techniques and how to combat some of these pitfalls. Dr. Duncan and Dr. Waltman both emphasize the importance and difficulty of learning these important skills. They described numerous resources for therapists interested in learning more about CBT, including websites, research studies, books, trainings, and certificates.

Dr. Waltman is a clinical psychologist, international trainer, and practice-based researcher. His interests include evidence-based psychotherapy practice, training, and implementation in systems that provide care to underserved populations. He is certified as a qualified Cognitive Therapist and Trainer/Consultant by the Academy of Cognitive & Behavioral Therapies and is board certified in Behavioral and Cognitive Psychology from the American Board of Professional Psychology. He is a board member for the International Association of Cognitive Psychotherapy. More recently, he worked as a CBT trainer for one of Dr. Aaron Beck’s CBT implementation teams in the Philadelphia public mental health system. He is the first author of the book Socratic Questioning for Therapists and Counselors: Learn How to Think and Intervene like a Cognitive Behavior Therapist.

She was past-president of the Association of Cognitive and Behavioral Therapies and the Society for a Science of Clinical Psychology. She is a Distinguished Founding Fellow, Certified Cognitive Therapist, and Certified Trainer of the Academy of Cognitive Therapy. She is the recipient of the Outstanding Clinician award from the Association for Behavioral and Cognitive Therapies and recipient of the Distinguished Contributions to Applied Research and Practice in Psychology award from the American Association of Applied and Preventive Psychology. She has authored 3 books and more than 70 articles and chapters.

Transformational chairwork is a transtheoretical psychotherapy technique which involves utilizing physical chairs in a session to parse out various “parts” of ourselves, speak from these parts, and allow our inner leader to gain control. Chairwork can also be used to role-play difficult conversations or as a part of grief therapy to reconcile any issues with individuals who have passed. Dr. Kellogg and Amanda Garcia Torres discussed the various components of chairwork and described how it can be used in conjunction with other psychotherapy modalities, such as cognitive restructuring in treatment for OCD. One of the core pieces of chairwork is about creating space for these various parts we may have been unable to hear through the chaos of everything else going on, or we have lost touch with. Amanda Garcia Torres also shares how chairwork can be used to treat issues related to social justice and traumatization from oppression. The episode ends with Amanda Garcia Torres leading Dr. Duncan through a chairwork role-play regarding feelings of anxiety. On this episode of Sanity, Dr. Jason Duncan and Dr. Paul Grant dive into Recovery-Oriented Cognitive Therapy, a new approach to cognitive therapy specifically designed to increase outcomes for those with serious mental illnesses, such as psychotic disorders. Dr. Grant explains how the focus of recovery-oriented cognitive therapy is on addressing negative beliefs an individual might have about themselves, others, or the future and building more adaptive “modes” of thinking by uncovering beliefs they have when they are at their very best. Through this treatment, individuals increase their sense of purpose and gain empowerment over their lives, which is often absent for those spending lengthy periods of time in psychiatric hospitals. This therapeutic approach is relatively new (past 10 years), however, initial study findings and past research on the various aspects of the model are promising. Dr. Grant also explains how this form of treatment can be useful for individuals with anxiety disorders, depression, or people who want to gain a deeper sense of purpose in their lives. On this episode of Sanity, Dr. Jason Duncan and Dr. Amanda Loerinc Guinyard discuss a new model of exposure treatment called the inhibitory learning model. Traditionally, exposure therapy was believed to work due to habituation, or the diminishing of a response to the feared or anxiety-provoking stimulus. Dr. Loerinc Guinyard describes how recent research suggests exposure therapy works because an individual is learning the previously feared stimuli no longer produces an anxiety response, known as the inhibitory learning model. Another aspect of this model includes an individual learning even if they are in an anxiety-provoking situation, they can tolerate and cope with those emotions. Positive Affect Treatment, a new therapy developed in part by Dr. Loerinc Guinyard, is also covered. This therapy is a transdiagnostic approach, meaning it can be useful for a number of different disorders, and its focus, not surprisingly, is on increasing positive emotional experiences, thoughts, and enhancing compassion. This is a great episode for those wanting to learn more about new therapeutic approaches and models!

On this episode of Sanity, Dr. Jason Duncan and Dr. Elizabeth Turk-Karan discuss how exposure therapy works, and how it is used to treat a number of different anxiety-related disorders. Dr. Turk-Karan shares how we all need some levels of anxiety to function properly, but sometimes, our anxiety can be triggered inappropriately. When this happens, exposure can be used to reduce the anxiety we feel. Dr. Turk-Karan explains how exposures are set up collaboratively with patients, and they include a hierarchy, from least to most anxiety-provoking. Patients are asked to “climb the ladder” and repeat these exposures for homework. Exposure works best when it induces sufficient anxiety, the patient’s anxiety levels drop, and when they are done repeatedly over time, not just in the office. As Dr. Turk-Karan explains, while this therapy sounds tough, trained therapists are there to support their patients every step of the way, and they would never ask a patient to do something they would not do themselves. Various types of exposure treatment are discussed, such as in vivo and imaginal, and numerous examples are provided. Dr. Robert Hindman on mindfulness for improving mental health

On this episode of Sanity podcast, Dr. Robert Hindman and Dr. Jason Duncan explore the benefits of mindfulness. Often times, when people hear the word, “mindfulness,” they think of meditation, deep breathing, and relaxation. But, as Dr. Hindman explains, it can be so much more than that. Mindfulness consists of two principles: bringing awareness to the present moment and accepting the experiences in the present moment non-judgmentally. Formal mindfulness is more similar to meditation training, whereas informal mindfulness can be practiced at any point or during any activity. Although Dr. Hindman states, “mindfulness is a simple concept that is difficult to describe or define,” he provides numerous examples of this intervention throughout the episode and even gives a short demonstration. In addition to allowing us to more fully experience the present moment, mindfulness can also be useful when implemented with other psychotherapy interventions, such as exposure. On this episode of Sanity podcast, Dr. Julie Orris and Dr. Jason Duncan discuss Dialectical Behavioral Therapy, or DBT. DBT was originally designed as a treatment for adults with borderline personality disorder (BPD) to improve emotional regulation and decrease harmful, impulsive behaviors. Dr. Orris discusses the importance of dialectics, or opposing concepts in DBT, and a core purpose of the treatment is to “find the middle path.” Since the foundation of DBT, it has been well-researched and is an effective treatment not only for BPD, but for other mental health concerns in adults and children, such as anxiety or depression. Dr. Orris explains DBT has four distinct elements: emotional regulation, distress tolerance, interpersonal effectiveness, and mindfulness. She discusses how mindfulness is taught in between each module because awareness of the present moment is an integral skill to DBT. At the end of the episode, Dr. Orris and Dr. Duncan discuss anxiety related to COVID-19 and how many of the fears we have are realistic. Using a DBT mindset, Dr. Orris encourages people to “find the middle path” of what reduces their anxiety about COVID-19 but allows them to live a meaningful life.
