USP Chapter 795: a review of changes for non-sterile compounding

The Compounding Center's pharmacists Jay Gill and Cheri Garvin discuss the changes prescribers and patients might see with their compounded medications because of the new standards going into effect November 1, 2023. The United States Pharmacopeia (USP) is the organization that sets standards for compounded medications, dietary supplements, and some foods. They have new guidelines going into effect that may cause some changes in compounded formulations and their beyond-use-dates (BUDs). BUD is terminology used in the compounding world, but most people think of it as the compounded preparation's expiration date. Compounded medications are designed to meet each individual's unique needs. For that reason there are hundreds of thousands of formulations. While each ingredient used in compounded medication is from an FDA approved manufacturer, the final preparation may not have gone through stability testing to know it's exact expiration date. For that reason, USP recommends very conservative BUDs. Cheri and Jay review the new beyond-use-dates for oral liquids, solid dosage forms like capsules, and topical creams and ointments. Managing Ehlers Danlos Syndrome with Low Dose Naltrexone

In this episode pharmacist Jay Gill interviews Dr. Norman Marcus, the Director of the Norman Marcus Pain Institute in New York. Dr. Marcus educates us about Ehlers Danlos Syndrome (EDS), a group of inherited connective tissue disorders. It involves every tissue of the body, but it tends to present in the skin, joints and blood vessels. With EDS, collagen is weakened or loose which leads to unusually stretchy skin or over flexible joints. Dr. Marcus uses the Beighton Score to help diagnose patients with hypermobility. This is just one of the things he looks for when considering a diagnosis of EDS. Patients with EDS typically present with joint pain, diffuse pain, fatigue, fragile skin or internal pain from blood vessels rupturing. Mast Cell Activation Syndrome or MCAS is a debilitating condition where a patient continues to experience allergic reactions, such as hives, swelling, low blood pressure, difficulty breathing, bloating, and severe diarrhea. TCC's pharmacist, Jay Gill, interviews Dr. Sheri Salartash from Dynamic Dental Wellness in Ashburn, VA. In this podcast Dr. Salartash educates us about sleep apnea. Did you know that 80% of patients with sleep apnea are never diagnosed!  Some signs the dentist may see when sleep apnea exists are cracked teeth, unusual wear on the teeth, recession around the gums, and jaw pain. These symptoms occur because the person is trying to move the jaw to get the tongue out of the way to breathe. Parents of children with sleep apnea may notice mouth breathing, dark circles under the eyes, and daytime sleepiness.   A diagnosis of sleep apnea requires a sleep study, but these can now be conveniently done at home.  Untreated sleep apnea can lead to decreased oxygen in the blood stream which causes our vital organs to suffer leading to significant health issues over time. When determining the best course of treatment for sleep apnea it's important to learn the cause of the sleep disorder. It could be a nasal obstruction, an upper airway obstruction, or central apnea which is a neurological issue.    Corrective therapies in dentistry aim to ensure the tongue has room to rest in the mouth so that it doesn't block the airway. Appliances or braces are used to expand the palate and jaw width. Tongue tie releases and/or tongue exercises may also be used. Another option for some patients is NightLaze. The use of low dose Atropine eyedrops for childhood myopia

Jay Gill, pharmacist at The Compounding Center interviews Dr. Kevin Chan, the Senior Clinical Director of Treehouse Eyes in Tyson's Corner, VA.  Dr. Gill and Dr. Chan discuss childhood myopia and its treatment. Childhood myopia is a condition where the eye shape elongates and causes nearsightedness or blurry vision when looking in the distance. Symptoms include: squinting, rubbing the eyes, blurry vision, and eye strain.  Unfortunately the incidence of childhood myopia is increasing. In 1970, 25% of the population was diagnosed with myopia. By 2050, 50% of population will become myopic. That means 1 in 2 children will need glasses or contacts.  Treatment consists of orthokeratology or contact lenses to help reshape the eye and low dose Atropine eyedrops. Dr. Chan points out that treating early is better. There is a fixed window to be treated for best results.  Dr. Gill explains how low dose Atropine drops are made. Atropine eye drops are commercially manufactured as 1% sterile eyedrops. If a doctor wants to order a low dose for a child's myopia, a compounding pharmacy like TCC dilutes the commercially manufactured drops to the dose needed. These eyedrops must be compounded or made in a sterile environment to avoid any potential for contamination. Once the 1% is diluted, the preservative in the original bottle is also diluted, so the bottle becomes important. Regular eyedrop bottles are open to the air and have the potential for bacterial or fungal contamination once opened. The Compounding Center uses special multi-dose eyedrop bottles designed specifically for preservative free eyedrops. The special one-way valve avoids accidental contamination from the air or from touching the eye or fingers. SIBO is more common than we think and is often underdiagnosed.  Up to 1/3 of patients with IBS have SIBO. Symptoms of SIBO are fat in the stool, vitamin deficiencies, bloating, gas, abdominal pain, weight loss, constipation and/or diarrhea. SIBO can also have non-gastrointestinal symptoms, such as fatigue, joint pain, headache, anxiety and depression. Dr. Delgado is passionate about providing a holistic approach to treating pelvic pain and helping women find relief. She teaches us that even though 25 million women suffer from this condition, many women come to her very frustrated because it is often misunderstood and misdiagnosed. Chronic pelvic pain is defined as pelvic pain that lasts more than 6 months. When you say pelvic pain most people think of endometriosis, ovarian cysts or fibroids, but it can have overlapping conditions, such as IBS, Interstitial Cystitis, and Pelvic Floor Dysfunction. Symptoms vary from vague lower abdominal pain before or during their menstrual cycle to stabbing pain or UTI like symptoms. Some women may also complain of chronic vaginitis, painful intercourse, anxiety and depression linked to their symptoms. Often times there is a history of trauma and there can also be a genetic component. When Dr. Delgado sees patients she wants to know and treat the whole patient and not just their symptoms. Initial appointments take approximately 1 hour to allow her to fully understand the patient's concerns. Dr. Delgado is a firm believer in tailoring the therapy to each individual person and to share the good news that there are always multiple treatment options.
