If the abs and lumbar spine are designed for stability, when doing an overhead reach in the sagittal plane, should the movement come from the thoracal cylinders since they should be the mobile part of the spine? Also when going in the front plane side to side, does the movement come from the thorax? Clarification would be most helpful. I work with women 50+ and the average age is 65. Thank you.

As you know in PT school we are taught to load certain musculature with one dimensional exercises to promote muscle hypertrophy such as weighted knee extension and heavy XXX squats to promote quadriceps hypertrophy. My question is for a bodybuilder athlete. How can a therapist/coach promote quadriceps hypertrophy utilizing a three dimensional approach rather than using a one dimensional approach such as knee extensions or by consistently overloading the XXX squat pattern.

Gary Gray discusses a question from Matthew Haley in regards to performing exercises with a pt/client with scoliosis. Do you encourage movement mainly in the pattern that goes opposite the curve to encourage new pattern and neuromuscular control and avoid moving in the already dysfunctional pattern, or is there some value in moving in old pattern (into the curve so to speak) in all 3 planes to encourage good mobility even in that pattern?
