How to track your cycle for fertility
You’re ready to start a family. Maybe you have a partner, maybe you don’t, but you’re ready. You just don’t know exactly where to start. Do you just start trying? Ultimately, this sort of thing can be sort of daunting, as anything new or different can be. Whether you’ve made the decision that you really want to start a family and are ready to do so right this second or you’ve been sort of trying to start a family for awhile and haven’t gotten pregnant yet, then you might consider tracking your cycle to get a better understanding of what’s going on in your body.

OHSS
Ovarian hyperstimulation syndrome (OHSS) is an excessive response to taking the medicines (especially injectable gonadotropins) used to make eggs grow. Rarely, OHSS can result from taking other medications, such as clomiphene citrate or gonadotropin-releasing hormone.
Women with OHSS have a large number of growing follicles along with high estradiol levels. This leads to fluid leaking into the abdomen (belly), which can cause bloating, nausea, and swelling of the abdomen. When OHSS is severe, blood clots, shortness of breath, abdominal pain, dehydration, and vomiting are possible.

Unexplained Infertility
Unexplained infertility is a diagnosis given to couples when all of the standard testing is normal but they have been unable to conceive for at least 12 months. This is a very frustrating diagnosis for couples,as they don’t know in which direction to go – whether they need expensive, invasive fertility treatments, or whether this is something that can be resolved naturally.

Premature Ovarian Insufficiency
POI occurs when a person’s ovaries produce fewer eggs. Often, doctors do not know what causes it, but it is often the result of an underlying condition. A doctor can diagnose POI based on a person’s medical history and blood tests. The treatment they prescribe will depend on the individual and whether they want to try to conceive. If they do, a doctor may prescribe hormones or medication to increase the chances of pregnancy, but many people will require donor eggs. Support groups or counseling may help someone cope with the emotional impact of this condition and its affect on fertility.

Endometrial Receptivity Assay (ERA test)
In this companion episode to Marguerite’s episode, I talk about ERA or Endometrial Receptivity Analysis.
The ERA (Endometrial Receptivity Analysis) is a genetic test that evaluates the expression of genes to evaluate whether the endometrial lining is properly developed to accept an embryo. This technology uses the latest scientific technology available to analyze the expression levels of the 236 genes to assess the optimal time to place an embryo into the uterus to promote a successful implantation and pregnancy. There is a small window of time that is optimal and in approximately 80% of women this window is in the expected time.

Mindfulness Series: Build Resilience by Practicing Radical Acceptance
To get really good at radical acceptance, we need to practice it, and fortunately life gives us endless opportunities to do this.
If you make a habit of accepting those small inconveniences where you have no control of the situation, you can build up your mental resilience so that you’ll be better equipped to accept a significant misfortune next time you’re unlucky enough to encounter one.
When you practice acceptance, you may still be sad, disappointed, or fearful. You don’t have to LIKE the situation you’re in and you don’t need to “try and look on the bright side”. You just need to welcome the experience and treat it with respect.

Mindfulness Series: Our Brain on Infertility
Our brains have been conditioned to grasp and cling. This conditioning, which we believe protects and helps us, actually limits our ability to live life fully. Now apply that metaphor to “our brain on infertility.” It’s the idea of a baby that we are grasping. And it’s understandable – having a baby is a primal urge. We clench with all our might around the idea that we want a baby and that we cannot be happy until we have one.
Meditation is a powerful technique for learning how to “unclench” the mind and find happiness that is not dependent on anything outside ourself. I encourage you to learn more about meditation and make it a part of your self-care practice today.

Mindfulness Series: Improve Fertility with Mindfulness
As part of my Mindfulness series, in today’s short solo episode I talk about how Mindfulness training is a safe and effective treatment with many benefits for women and men undergoing infertility, particularly IVF.
Anxiety and stress may actually make it more difficult to conceive. In fact, recent studies found that higher rates of infertility treatment success in people who were undergoing a mind–body interventions. So, reducing stress and anxiety may be beneficial for conception. Since mindfulness training has been shown to reduce anxiety (see http://contemplative-studies.org/wp/index.php/category/research-news/anxiety/) and stress (seehttp://contemplative-studies.org/wp/index.php/category/research-news/stress/) it is reasonable to believe that mindfulness training may be helpful for conception in women with fertility issues.

She went through IVF but this resulted in a partial molar pregnancy. She then decided on surrogacy and through her surrogate was able to have twins!

Coping with Pregnancy Loss
Coping with Pregnancy Loss
1. Understand that it's not your fault. Pregnancy loss or complications can strike anyone. Talk openly and honestly with your partner about what's happened and how it's affecting you. Remember, there's no right or wrong way to deal with grief. Accept your feelings as they are and don't judge yourself or your partner for how you respond.
2. Give yourself time to heal. Don't pressure yourself to get past the sadness quickly. Your healing will be more complete if you deal with your grief as it comes. You may find yourself reliving the pain, especially around your due date or other milestones. Over time, things will change and you'll feel better.
3. Take time off from work. Even if you feel physically fine, taking some time away from your job may be helpful. You need a chance to process what's happened, and taking a break from your regular routine will help you acknowledge and accept all that you're going through.
4. Don't expect your partner to grieve in the same way. I
5. Don't close yourself off from others.
6. Seek out professional help through a counsellor or join a support group near you.

After being told she has a less than 1% chance of having children, her FSH was elevated and was in the “pre-menopausal” range. She was told that adoption or egg donors were the only other option. Laura didn’t take no for an answer. She sought the opinion of multiple REs, she researched, worked with a naturopath and an acupuncturist. She cleaned up her home environment and her diet. She was able to decrease her FSH naturally. After 4 rounds of IVF, her daughter was born without the use of an egg donor.

“Allow yourself to walk through negative feelings. When we go through IVF, we have these negative fears like “what if it doesn’t work?”, and we have anxiety and depression, especially in the two week wait. All of these bring a lot of stress to our bodies. Don’t hide those feelings. They will pile up and it is worse. Run, scream, curse, cry.. Go to a support group. Allow yourself to feel that. Then let it go. Do art therapy. Watch movies, watch something funny. Do not forget about your partner, that there is a relationship there. We have a tendency that when we go through this, and our focus is baby-baby-baby, we forget our world around us and that we had a life before. We still need to stay connected.” -Monica

Today’s guest is Marguerite from Virgina. She is a lawyer, infertility warrior, and dog mom. She has been married for 9 years and have been TTC since the beginning. She was diagnosed with prolactinoma, endometriosis, and diminished ovarian reserve. She first tried NaPRO. They tried a timed and medicated intercourse with Letrozole but was unsuccessful. Marguerite also developed ovarian cysts as a side effect of Letrozole. She and her husband then tried IVF. She had 2 egg retrievals back to back. But they decided to postpone the fresh transfer due to her husband’s diagnosis of COVID. They have frozen embryos that they will wait later in the year to transfer.

Hypothyroidism and Fertility
As a companion to Monique’s episode, Mary dives into Hypothyroidism and how it affects fertility.
Hypothyroidism is a leading cause of difficulty in achieving and maintaining pregnancy. Low thyroid function has a negative impact on reproductive health and is more common than most women realize.
When your thyroid gland isn’t making enough thyroid hormone, it can have a serious effect on every organ in your body — including your reproductive system. This means hypothyroidism may make it difficult to conceive if you’re planning on having children. A study published in August 2015 in the Journal of Pregnancy found that women with hypothyroidism were less likely to become pregnant — and more likely to take longer to become pregnant — than women without the condition. That’s because women with hypothyroidism may not ovulate or ovulate with any regularity, and you have to ovulate to get pregnant.
Hypothyroidism can affect fertility in men as well, according to the Thyroid Foundation of Canada. Although hypothyroidism is less common in men, those who do have an underactive thyroid may have low libido and low sperm count, according to a review of research published in November 2013 in Frontiers in Endocrinology in November 2013.
If you’re experiencing symptoms of hypothyroidism — fatigue, increased sensitivity to cold, constipation, dry skin, weight gain, muscle weakness, and heavier than normal or irregular menstrual periods in women, among others — you should get tested for the condition. A simple blood test can reveal whether your thyroid gland is functioning normally. The test measures the amount of thyroid stimulating hormone (TSH) in your bloodstream, and high levels suggest hypothyroidism, according to the American Thyroid Association.
Hypothyroidism is one of the many common conditions that your doctor can test for as part of a pre-pregnancy health check, according to the American Pregnancy Association, noting that it’s better to identify and treat health issues before conception. A conversation about pre-pregnancy testing is especially warranted if thyroid health problems run in your family, the British Thyroid Foundation says. Women who have difficulty getting pregnant or who have had miscarriages previously should be tested
When hypothyroidism is the reason for infertility, taking thyroid medication will enable most women to conceive, from as soon as six weeks after treatment, according to a study published in February 2015 in the IOSR Journal of Dental and Medical Sciences. The study also pointed out that many women who have a problem conceiving may have no apparent symptoms of hypothyroidism and only slightly elevated TSH levels, making it all the more important to have a TSH blood test if you’re having a hard time getting pregnant and don’t know why.
Another study found that treating hypothyroidism with medication not only improved conception rates, but also reduced miscarriages early in pregnancy, which can happen as a result of untreated severe hypothyroidism. Those results were published in January 2015 in The Obstetrician & Gynaecologist.
It is best to see your doctor if you think that you may have hypothyroidism and further blood work can be done to investigate.

Monique
Today’s guest is fellow fertility podcaster, Monique Farook, host of the Infertility and Me podcast. She is an infertility survivor, former NICU Mom, wife, and serial entrepreneur. She and her husband began their path to parenthood in the Spring of 2012 when she was 29 years old. After completing the HSG test to determine if her reproductive organs were in good health, the radiologist concluded that she had a right tubal blockage. After one unsuccessful IUI (Intrauterine Insemination) she and her husband waited four years before seeking the expertise of a reproductive endocrinologist for a second time. It was then when she was diagnosed with hypothyroidism and uterine polyps. Although the right tubal blockage had "cleared" itself, she had two new unforeseeable issues to deal with before she could begin any form of IVF treatment. In August of 2016, Monique became pregnant with her baby boy after one failed cycle. In January 2017, Monique went into preterm labor at 23 weeks five days and remained in the hospital on bed-rest for an entire week. At 24 weeks four days pregnant(6 months), her baby boy was born and spent 129 arduous days in the NICU. He is the inspiration behind Monique’s passion for infertility advocacy.

Polycystic ovarian syndrome (PCOS) affects 5%-10% of all women of reproductive age making it the most common endocrine disorder among women. While the exact cause of PCOS is still not clearly identified, the majority of research implicates having excess levels of insulin or insulin resistance as its main cause.
In fact, 8 out of every 10 women with PCOS could have insulin resistance, resulting in higher than normal insulin levels. This increase in insulin decreases sex hormone binding globulin (SHBG), which increases androgens (testosterone). This has a direct effect on ovarian cyst formation. The end result is that a PCOS sufferer may stop ovulating, gain weight, develop skin conditions like acne and skin tags, and grow abnormal facial and body hair. The clinical picture of PCOS varies widely from woman to woman, and because the symptoms vary so widely and not all women display all the symptoms, doctors often misdiagnose PCOS. It is best to see your doctor if you think that you may have PCOS and further blood work and ultrasound and can be done to investigate.
